Book Abstract

由于大多数抗癫痫药物都有嗜睡和镇静作用，所以服用抗癫痫药后可能引起急慢性精神障碍。急性发作多表现为头晕、嗜睡、惊恐、易激动、心情抑郁、烦躁不安，这些症状多在用药1~2周内出现；慢性发作后的意识模糊时间长，语言困难，思维过程缓慢，注意力和记忆出现障碍，慢性反应是由于药物的慢性毒性作用所造成的，患者多在用药数年后出现，且药物减量或停药后会逐步消失。
并非所有服用抗癫痫药物的患儿都会出现精神障碍，是否出现这些情况，与癫痫的发病年龄、发作的次数、癫痫的类型、服药种类、和所服药物的多少等因素有很大关系。患儿如何更换抗癫痫药物？
抗癫痫药物的更换过程很重要，也很讲究。换药时必须在第二种药物达到起效剂量和浓度时，才能把第一种药物逐渐减停。以免出现治疗的空窗期，导致癫痫持续状态的发生。主要有以下两种方案。
第一种是原来的药物按原剂量服用，同时选择一种适应于该发作类型、毒副作用也相对较小的新药物，把新药计算总量后分4步加服，一周增加一个剂量，如果用药后无明显的副作用且取得了比原来更好的疗效，则逐渐减去原来服用的药物，如减去原用药疗效减弱时，则应采用两药合并治疗。此种方案的优点是能观察两种药物的联合作用，缺点是两种药物的副作用会累加。此法可免去患者因突然换药而致的癫痫发作以及开始用药因剂量过大而致的毒副作用。
另～种方法是，当确定第一种药物效果不佳时，换用另一种一线抗癫痫药物，逐步加至合适剂量。然后渐渐停用初用的抗癫痫药物，仍用单药治疗。替换需根据药物半衰期及达稳态浓度所需时间来决定。达稳态血浓度所需时间一般5-7倍于药物半衰期，所以半衰期愈长，则血药浓度达稳定状态所需的时间也愈长，反之亦然。如果撤药过快，以免血中浓度不足而使发作加剧。但对发生过敏反应或造血障碍者，应立即停服该药，以免酿成严重后果。癫痫在何种情况下可以考虑联合用药？
合理、正确的单药治疗可使80%左右的癫痫患者发作获得有效控制，但仍有一部分癫痫患者需要联合用药才能控制发作。多药治疗可能增加药物的不良反应，也加重了患者的经济负担。因此新诊断的癫痫患者尽量单药治疗。和换药治疗前一样，不要轻易的判断一种单药治疗是无效的。应该确定前一次单药治疗是否依从了正确的方法。
