Book Abstract

1.知常达变，有独到见解，发前人所未发傅氏在前人妇科理论指导下，崇经而不泥经，勇于独创，敢立新说，谈证不落古人窠臼，对每一证的论述，都先叙常人之认识，后抒自己之见解，并举一反三，加以论析。在临床实践中，既遵古训，又灵活施治。他在《题幼科证治准绳》中说：“既习此，实无省事之术，但细细读诸论。”认为前贤的宝贵经验，必须虚心研读，精心领会，而“不可不知其说”。但在证治时，则应具体分析，有取有舍，灵活运用，绝“不可尽倚其说”，生搬硬套。例如在《女科·调经》中，其定经汤、调肝汤、两地汤、温脐化湿汤和健固汤等诸方剂的组成，都常有变，随证加减，说明他一面继承古训，一面吸取新知，从而创制出行之有效的方剂。《女科》中有些论点简明扼要，且中肯綮。如带下病篇首即提出“带下俱是湿症”，一语千钧，为临床治疗带下提供了充分依据。再如对经水过多一证，一般多从血热或气虚论治，而傅氏则认为是“血虚不能归经”，并辨证地指出“血归于经，虽旺而经亦不多，血不归经，虽衰而经亦不少”，为治疗经崩漏下拓宽了思路。对月经“赤红似血而实非血”，“乃天一之水，出自肾中，是至阴之精而有至阳之气”的见解，破世俗之误而更高一着，因为月经除了属血的一面还必须有肾气的参与，倘无肾气施化则经水断不能依期而行止。可见傅氏敢于创新补先人不足。在病因方面，除了内外因的作用，傅氏还十分强调“房室不节”这一易为人们忽视的致病因素，认为“年老血崩”、“少妇血崩”、“交感血出”、“行房小产”、“产后血崩”等都是由于“不慎房帏”之过，并告诫人们“血管最娇嫩，断不可以精伤”，合之非道，精伤气耗，动火动血，为害非浅。这不仅示后学治病时应探微索隐，详察病因以正确治疗，而且对妇女四期保健，防患于未然，也有积极地指导意义。
