吴中朝，中国中医科学院教授、博士研究生导师，中央保健会诊专家，中国中医科学院针灸医院主任医师。

Book Abstract

让艾灸事半功倍的4个方法
要想让艾灸发挥应有的效果，除了掌握艾灸的基本方法，还应当了解一些前人艾灸的经验，这样才能让艾灸事半功倍。
◆多灸背，少灸胸古人艾灸一般多选择背部穴位，而避免灸胸部。这是有一定道理的。明朝医家李梃在其著作《医学入门》就提到，肌肉偏薄之处、骨骼之上，以及大血管和活动关节、皮肤皱纹等部位，应避免采用直接灸。一般来说头部、面部、胸膈以上的部位不宜多灸，而背部、
下肢等肉厚部位则多灸无妨，尤其是背部腧穴多灸常灸无妨。背部上有许多重要的腧穴，如肺俞穴、心俞穴、膈俞穴、肝俞穴、气海俞穴、关元俞穴、三焦俞穴等。这些穴位与五脏六腑一一对应，所以多灸背部可祛病缓疾、延年益寿。
◆文火补，武火泻古人根据灸疗的温度和方法，将艾灸之火分为文火和武火两种。文火指的是火力小而缓，一般来说，温灸盒、麦粒灸等都被视为文火；武火即火力大而猛的火，大艾炷即为武火。文火和武火的艾灸作用是不同的。古人认为“文火为补，武火为泻”。明代杨继洲在《针灸大成》中说：“以火补者，毋吹其火，须
待自灭，即按其穴，以火泻者，速吹其火，开其穴也。”意思是说，凡火力由小到大，不需要吹灭而使其慢慢燃尽者为补法，能起到温阳补虚作用；如果将火吹旺使病人有烫的感觉，则为泻法，能起到驱寒散结的作用。所以在艾灸时，我们不应一味追求武火，追求刺激，而应当根据病情或目的来选择合适的温度
◆先阳后阴，先上后下“先阳后阴，先上后下，先少后多”是灸疗操作的常规。《千金方》中就指出：“凡灸当先阳后阴……先上后下。”临床操作一般先灸上部、背部，后灸下部、腹部；先灸头身，后灸四肢。“先阳后阴”的目的是达到阴平阳秘，而无亢胜的弊端。“先上后下”就是先灸头面躯干部，后灸四肢部，或先灸头面与胸部，后灸腹部和下肢部。因半身以上同天之阳，半身以下同地之阴，这样艾灸可以达到阴升阳降，水升火下，水火既济。“先少后多”就是初灸者刺激量宜先小后大，以便患者逐渐适应。
◆选好时段效果好古人将一天分为12个时辰，分别对应十二经络，每一个时辰有一条经络主要工作，所以要想通过艾灸解决某条经络上的问题，好能与经络工作时间相对应，效果更好。当然，有些经络当令时间在夜间，可不必拘于时段。
