贝尔纳·格朗热（Barnard Granger），精神科医生和心理治疗师，法国认知与行为治疗协会会员，巴黎笛卡尔大学教授，塔尔尼耶-高善医院精神科负责人。他在Cavalier Bleu出版社出版的著作有《抑郁症》和《精神分裂症》。

如果有人问“我患了抑郁症吗？”，他不一定能得到明确的答案，因为抑郁症的表现形式多样又具有欺骗性。
抑郁症在变为长期和彻底之前，最开始可能就只是一个反应型的忧伤感。人们在日常生活中很容易把忧伤的轻微形式和过度阶段称为消沉。
相反，还有一些强烈的抑郁症被称为忧郁，这个词以前被用来指代所有的抑郁症。忧郁症的特征是强烈的精神痛苦和出现谵妄性观念：病人认为自己被剥削被迫害，或者把一些他不曾犯过的错误揽在自己身上。忧郁症还可伴有科塔尔综合症（Cotard syndrome），它妄想器官部分或全部不存在了：患者坚信自己的某个器官不能正常工作了。最极端的，患者认为自己已经死了或正在地狱里受苦。
抑郁症随年龄不同会有不同的表现。小孩子就是学习成绩差，不爱玩、爱淘哭。青少年则表现为带有危险的行为，如神游症、嗑药、吸毒（大麻、可卡因、海洛因、酒……）。老年人的抑郁症表现可能是精神错乱的先兆：思维混乱、精力无法集中、记忆力差。但与精神错乱（如阿尔茨海默病）不同的是，老年人的抑郁症更多的是突发性的，一旦使用抗抑郁药就能改善。
女性一生中有几个阶段最容易得抑郁症。产后的几天，通常母亲都感到易怒和忧伤，即产后忧郁。这种状态在48小时至72小时就会消失，但是，如果持续到产后几周，出现了一系列的抑郁症症状，我们就管这叫产后抑郁症。妈妈总感觉应付不了她所面临的任务，或者感觉自己没有照顾好婴儿，是个“差妈妈”。有一些产后抑郁症患者出现发狂状态，在极端情况下有可能杀死自己的孩子或自杀。绝经期得抑郁症患者的可能性也较高。这个状态很可能跟这个生殖器官在此阶段引起的心理和生理调整有关。
某些时候，有些患者的症状具有抑郁症的性质，但临床表现不足，我们称之为少症状的抑郁症，意思是抑郁症的症状较少。但也有些患者的抑郁症比较显著，表现为症状，如疼痛或其他的身体不适，不过情绪没有明显改变。当然临床检查还是能找出其他较不明显的抑郁症候因素。
抑郁症状态的发展过程也各有不同。通常抑郁症首次发病后经过六个月到一年可能不治而愈。但是反复性短暂抑郁发作和慢性抑郁症就不符合这一发展模式了。反复性短暂抑郁发作每次发作都很猛烈但历时很短，只有几天但是经常反复发作，慢性抑郁症发作期长，可能长至几年，但通常病情不如反复发作的重。
最后是季节性抑郁症，它总是在一年中同一时期发作，通常是日照大量减少的时候（北半球地区的秋季和冬季）。这些季节性抑郁症患者通常嗜睡并偏爱甜食。
抑郁症的误诊有可能是没有查出来，也有可能是过分夸大。不是所有的精神病都是抑郁症，很多判断和诊断都是值得推敲的。由于大多数精神障碍在某个发展期都具有抑郁症的症状，这就增加了抑郁症鉴别诊断的难度。
首先要把抑郁状态和情绪的心理变化区分开，不论后者是自动产生的（不知出于什么原因在一段时间内感到一种不满足或忧伤的情感）还是出于对一些悲伤恐惧事件的反应。最明显的例子就是服丧期，这时的反应在好几个方面都堪比抑郁症：忧伤、对生活感到厌倦、丧失乐趣、不断地想念亡者，甚至想随之而去，没有胃口、睡眠障碍。不过这种反应是暂时的，慢慢会恢复常态。
抑郁症的状态中还常常伴随着焦虑，但是这类焦虑也可能仅是焦虑症，包括惊慌失措、急性焦虑发作、长期焦虑、恐怖症和强迫性紊乱。在某些生活的艰难困苦中，如工作上的冲突、失业、离婚、亲友的重病、人们也会经历反应性焦虑，其特征是失眠、注意力难以集中、心中烦恼不断、某种程度的忧伤和伴随呼吸困难的心悸。
某些身体上的疾病可以导致严重的厌倦疲乏和注意力不集中。一些内分泌疾病患者会出现这种情况，如甲状腺机能减弱或癌症。看到身体和精神上的疲乏时，还要找找身体上的原因才能诊断是不是抑郁症。
这种诊断后果还不严重，抑郁症还有可能被误诊为其他精神错乱，如精神分裂症和谵妄。但它们之间的区别很大，首先要看有没有出现幻觉和谵妄性观念。而且精神分裂症患者完全错乱，且部分地脱离现实。
最后，不能把所有与外界环境或人格障碍有关的精神不适都叫做抑郁症。如法国心理学家米歇尔？勒儒尤教授（Michel Lejoyeux）写道的：“莫里哀剧中的那个没病找病的人把他所有的病都归咎于自己的身体；新近出现的多愁多虑的人认为自己患上了抑郁症或焦虑症，但没有任何诊断能予以证实。”
抑郁症的诊断通常很复杂，需要丰富的临床经验和对病人精神状态进行深入评估。日常语言尚且试图分辨“消沉”和“抑郁症”，这简单反映了在医疗领域中诊断所有精神病（特别是抑郁症）常会遇到的困难。
