在决定第3版《体外循环原理与实践》改编之前，我们的编者们一直被当前日益增多的非体外循环下冠状动脉搭桥手术和经皮行心脏瓣膜置换术等现象所困惑，究竟现在还是否需要出版有关体外循环的理论书籍呢?很显然，我们最终的一致意见是：体外循环的现状和未来都很美好，而且现在也是出版第3版《体外循环原理与实践》的最佳时机。尽管某些需要体外循环的外科技术在逐年消减，但仍然还有很多医学领域在开发和应用体外循环及辅助支持技术。目前在先心病外科领域就已应用心室辅助装置针对衰竭心脏进行重塑和长期辅助支持，并取得了可喜的成绩。对于我们的高龄人群中患有瓣膜疾病的患者，采用外科成形或瓣膜置换手术依然是治疗的最佳手段。在儿科心脏外科团队中的老、中和青年学者们都在努力地推动先心病矫治术的不断前进和发展。这些都需要我们具有十分丰富的体外循环管理经验。近年发展起来的微创外科技术可以承诺患者缩短住院时间并能快速恢复日常活动，但这项技术的绝大多数患者都需要在体外循环下完成，而这些体外循环已发生了显著的变化：技术更加精良，设备更加小型化。在1993年本书的第1版中所述及的体外循环对机体生理功能方面的不良影响，都已被今天体外循环技术和设备的发展所改善。但随着诊疗患者群中老龄化和危重病种的不断增加，我们仍需要在体外循环领域不断地开拓进取。

第一篇体外循环发展简史
第1章体外循环的发展史——明尼苏达州
早在1952年，内科医生面对因心内畸形而生命垂危的儿童，除祈求其能康复外而别无他法。现在，借助心肺机，心脏畸形矫正已成为常规手术。因此，心脏直视手术被公认为是20世纪医学界最重要的进步之一。仅仅40年前人类的心脏外壁还是外科医生的手术刀难以逾越的解剖屏障，而今心脏直视手术开展的如此之广泛（全世界心脏直视手术2000例／24小时），手术操作如此之简单且对各个年龄段的人群来说风险如此之低，取得的治疗成就如此之大，令当代的心脏病学家和心脏外科医生都感觉不可思议，更不用说普通民众了。
通过体外循环进行心肺转流是取得这一惊人进步的基石。通过体外循环技术使得外科医生可以排空心脏中的血液、根据需要使心脏停跳、切开任意的心腔、安全实施修补操作甚至从容地进行全心置换术。
从1951年开始，明尼苏达大学的外科部门取得一系列进展，从而使心脏直视手术从实验室研究转变为临床成为可能并且取得成功（表1.1）。该机构具有引以为豪的两大资本：一是拥有世界上第一所专科心脏病医院，共有80张儿童和成人床位，由Northwest的Variety俱乐部捐赠给明尼苏达大学并于1951年7月1日对病人开放；二是拥有Owen H.Wangensteen医生（此点也许更加重要），他是一位真正有远见的外科医生，担任外科部门的主任。他虽然不是心脏外科医生，但他在肠梗阻的预防和防治方面的创新性工作对普通外科学领域做出了巨大的贡献。
