Author Description

李邻峰（李林峰），教授现任北京大学第三医院皮肤科主任，北京大学第三医院皮肤性病学研究室主任，北京大学皮肤性病防治中心副主任。博士生导师。毕业于北京医科大学。1992年在北京医科大学研究生院获医学博士学位。1995～1998年在美国伊利诺大学皮肤病学系及遗传学系任访问副教授，并于1997年聘为研究员。从事接触性皮炎及脱发病研究。曾获美国芝加哥皮肤病协会研究基金奖。1998年回国工作至今。主编著作包括：《皮炎湿疹的发病机制》，《皮炎湿疹的临床诊断》，《皮炎湿疹的治疗》，《接触性皮炎与皮肤变态反应》，《接触性皮炎》，《皮肤性病学》，《肾上腺糖皮质激素在皮肤科的应用》及《全微助忆医学复习精要》。参加编写著作多部。发表中英文论文80余篇。积极参与医学继续教育及科学普及工作。发表医学科普文章数十篇。1995年曾获《健康报》“药物不良反应”有奖征文二等奖。自1994年起，已主讲了11期全国接触性皮炎与皮肤变态反应学习班。兼任中国医师协会皮肤病分会委员，中华医学会北京分会皮肤科委员会委员，中华医学会皮肤病分会变态反应学组委员，中华预防医学会职业病专业委员会皮呋病学副组长，中国预防性病与艾滋病基金会理事与专家组专家，《中国麻风皮肤病杂志》编委，中国性学会性传播疾病防治委员会副秘书长，全国卫生专业技术资格考试专家委员会委员。

Book Abstract

3.对于斑贴试验，糖皮质激素类药及免疫抑制剂应该停药2周以上。对于速发型反应的皮肤试验和血清特异性抗体检测，应在紫外线照射后（如强烈日光照射后）2～3周再测试。强紫外线照射后4周内勿作斑贴试验。
二、结果解释注意事项
无论何种诊断试验，阳性结果只能说明患者存在对该变应原的致敏状态，至于该过敏原是否是患者皮肤病的病因还不清楚。做完检查试验后，应该仔细询问病史，如果患者每次发病（至少2次）均与接触该变应原有关，则可以确定该过敏原是其皮肤病的病因。如果病史不能确定，则需要让患者先避免该变应原一段时间，待皮损消退后进行激发试验，如应用试验等，判断是否激发出皮损，阳性则可以确定因果关系。有时，该过敏原只是患者现有皮肤病的加重因素。还有相当多的情况是，敏感过敏原是患者过去患皮肤病的病因，如有金属过敏史的患者又出现了染发皮炎，在以后的测试中可以测试出金属过敏，但不是当前皮肤病的病因。过敏试验检测阳性结果与临床的关系可以分为以下几种：1.现在相关：阳性过敏原是现有皮肤病的病因或加重因素。
2.过去相关：阳性过敏原是患者过去曾患皮肤病的病因或加重因素。
3.将来相关：阳性过敏原在检测时没有发现相关性，但在将来随访中出现了与之相关的皮肤病。4.相关性不明：不能确定患者是否接触该过敏原。

Introduction

湿疹皮炎类皮肤病是皮肤科常见病，美国2007年调查一般人群患病率10.7％，同年，我们调查发现我国社区人群患病率约为7.5％。由于病因复杂，反复复发，也是难治性皮肤病。此类疾病被许多人称为“三无”病：①无生命之虞；②无根治办法；③无特定病因。或被称为“三不”病：①不明白；②死不了；③治不了。实际上，只要我们深入细致地了解此类疾病，湿疹皮炎类皮肤病还是完全可以控制的。反之，如果我们不去深入了解每个患者的具体情况，盲目追求快速、彻底地消除症状，使用副作用过大的药物，死亡的病例也不少见。
湿疹皮炎类皮肤病常被认为是变态反应，即过敏反应，但并非所有湿疹皮炎都是由过敏引起的。因此只有使用合理的诊断方法，正确诊断，才能达到彻底根治皮肤过敏的目的。否则，盲目诊断变态反应，不加区别地盲目泛泛“忌口”，不但达不到治疗目的，还可能适得其反，造成机体营养失衡，免疫紊乱，最终加重疾病。
