Book Abstract

有小耳畸形的人，多合并外耳道、中耳及小听骨畸形，因此听力多数不好，一般属于传导性耳聋。如果两耳都是畸形，听力都受影响，轻症的可配戴合适的助听器，重症者宜先做一侧手术。
外耳道完全闭锁者，外耳道充满骨质，或呈膜状，其口部只有浅的凹陷，常缺少鼓膜，听骨链中断或关节僵直（锤骨与砧骨融合，或镫骨固定），失去了传音功能，表现为先天性传导性耳聋，双侧者可发生语言障碍。
治疗：根据畸形的程度，做外耳道成形术或鼓室成形术。手术前必须行X线拍片检查，以明确乳突的气化和中耳、内耳发育情况。听力和前庭功能也要检查，以了解听力损失、前庭功能情况。
手术的目的是提高听力和整形。手术的年龄，如果双侧外耳道闭锁，以5～6岁手术为宜，先做一侧，及早提高听力，便于孩子人正常普通小学学习。如单侧，待年龄大些再做手术也可，以便提高听力，恢复辨别声音方向的能力，有利于安全。
5.耳廓外伤怎么办？
耳廓突出在头部的两侧，最容易受到外伤，如切伤、挫伤、撕裂、挤压伤、冻伤、咬伤等。一般小的皮肤损伤，要处理及时，局部用生理盐水清洗干净，涂上少许红药水（红汞液）或碘酒、酒精。这些药消毒杀菌作用都很好，但用后稍痛一些。根据需要可用消毒纱布包扎伤处，防止细菌感染。一般数日可自然痊愈。
撕裂伤：轻者仅有裂口，重者有组织缺损，甚至耳廓被全部撕脱下来，应在严密消毒下，按外科原则进行缝合。
