Author Description

姚春海，中国中医科学院西苑医院皮肤性病科主任、副主任医师，北京中医药学会皮肤病专业委员会理事、世界中医药学会美容专业委员会理事、北京中医药大学客座教授。1989年毕业于北京中医药大学，一直从事皮肤性病的临床诊治、科研及教学工作，对皮肤性病科常见病、多发病的诊治具有独到之处。善于应用中西医结合治疗各种急慢性湿疹、特应性皮炎、痤疮、银屑病、荨麻疹、性传播疾病等。承担国家、北京市、医院课题多项，发表论文论著30余篇。连续数年代教欧洲留学生、进修生、实习生，并多次做专题讲座。近年来，负责西苑医院多个西学中班、港澳台班的皮肤病学教学工作。

一、概述 1.什么是皮肤瘙痒？皮肤病的症状和体征，是认识和诊断皮肤病的重要依据。所谓体征，是指医生检查时所见到的各种皮肤损害，如红斑、丘疹、结节、水疱、脓疱、糜烂、结痂及鳞屑等。症状则是指患者的主观感觉，主要有瘙痒、疼痛、麻木、烧灼感和蚁行感等。其中，以瘙痒最为常见。瘙痒是一种可引起搔抓欲望的皮肤感觉。它可呈阵发性或持续性，可局限于身体某一部位，也可泛发全身。不同的人对瘙痒的反应有所不同，痒的轻重程度也因身体不同部位皮肤及皮肤的状态而有差异。例如，肛门、外阴部皮肤对痒特别敏感，皮肤干燥易引起瘙痒。引发瘙痒的因素很多，外因有气候的变化，寄生虫（如疥虫）的活动，昆虫的刺蜇，某些药物、食物、饮料或炎性渗出物等；内因有精神紧张，皮肤温度升高，皮脂腺分泌减少，细胞内核糖核酸增加，肥大细胞增多，脑脊液中钾、锂、锶等离子增加等。瘙痒是一种症状，不是病。它可见于湿疹、接触性皮炎、荨麻疹、药疹、脂溢性皮炎、扁平苔藓、疥疮、虱病、蕈样肉芽肿等多种皮肤病；某些全身系统性疾病，如糖尿病、胆汁阻滞疾病、内脏恶性肿瘤、尿毒症、甲状腺功能障碍、月经不调、闭经等，也可引起瘙痒。
