Book Abstract

（二）标本兼治，立足治本
对任何一种疾病的治疗都需要从治标与治本两个方面入手。在一般情况下，我们都采取急者治其标、缓者治其本或标本同治的治疗方案。所谓治标，即是对症治疗，以解决患者的疾病痛苦为主要目的，可能对疾病的致病原因起不到很大作用，但是，对缓解疾病造成的病痛却是十分必要的。所谓治本，即是对致病原因进行根本性的治疗，对彻底治愈疾病起到决定性的作用。从理论上讲，医生治病就应该从治疗病因开始。可是，在医疗实践过程中我们会常常遇到有些疾病可能一时难以找到确切的致病原因，或者知道病因，却因种种条件限制一时无法进行满意的病因治疗；或者患者的疾病痛苦十分剧烈，已经成为当时治疗疾病的主要矛盾等。在某些特殊情况下，甚至暂时放弃病因治疗而重点进行对症治疗。作为一名医生必须明确地认识到，治疗疾病的根本目的在于彻底将疾病治愈。为此，最终治疗方案的选择，一定要立足于对疾病的病因治疗，切不可只满足于对症治疗取得的效果。
（三）综合治疗，突出重点
随着医学的发展及对疾病治疗方法的不断增加，对任何一种疾病都有若干种治疗方法。面对多种多样的治疗方法，我们在选择药物治疗方案时，要特别注意综合治疗，防止忽视实际需要和客观条件，过度地追求大而全的治疗方法。应该紧紧地联系自己的工作条件与患者的客观需要，从众多的治疗方法中选择适合患者与诊所条件的治疗方法。防止忽视社会心理因素和护理治疗，单纯地考虑药物、手术或其他技术疗法。基层医生以家庭为治疗单位开展疾病防治工作，有更多的时间接触患者及其家属，自然对患者的心理和社会影响了解得较为深入，为较好地进行心理治疗奠定了基础，这是在医院工作的专科医生所无法比拟的。此外，家庭治疗与护理常常是密切结合在一起进行的，有时两者难以截然分开，所以，护理的治疗作用理所应当地划归在治疗方案之中。在一个治疗方案中包括几种治疗方法，必须抓住重点，明确哪一种治疗方法是主要的，是矛盾的主要方面。同时在治疗过程中还要依据病情变化，及时地调整治疗方案，适时转移治疗重点。
（四）简单易行，防避风险
医生在制订治疗方案时，在有效果相近的多种治疗方法的情况下，尽量选择那些简单易行、风险较小的治疗方法。就是治疗方法要能用简单、方便的，就不用复杂的；能用青霉素治疗的就不用广谱抗生素治疗；能用口服治疗的就不用肌内或静脉注射治疗。同时还要考虑药物治疗方法的风险性，对患者能有多大损伤、危险，可能会导致什么结果。尽量采用那些对患者损伤小、不良反应少的药物治疗。如果一定要采用不良反应或毒性较大的药物，也要向患者及其家属详细交代用药后可能出现的不良反应，并说明用药期间的注意事项以及出现不良反应时的处理办法。

第一版《全科医生药物手册》自出版发行以来，因体现了简单、精练、实用的特点，经过广大基层医务人员的实际使用而颇受好评。本次再版是以我国2013年发布的最新的2012年版国家基本药物目录为基本内容，并结合乡镇卫生院、村卫生站、街道医院和厂矿企业医务室等基层医务人员工作中的实际需要，补充了基层医疗需要的一些常用药。本书的主要特点是包含药物广、实用性强。为了使基层医务人员更好地使用药物，不断提高药物治疗水平，在手册的开始部分简要介绍了药物治疗学的基本原理和药理学基本知识。本书的重点放在基层常用药物的临床应用和主要不良反应及其防治原则。为了促进合理用药，减少药物不良反应的发生率，本书对药物的不良反应和应用注意做了尽可能详细的介绍，同时对部分老药的最新临床应用也进行了适当补充。书中带有④符号的药物为最新国家基本药物（共567个，其中包括耐多药肺结核用药、国家免疫规划用疫苗、抗蛇毒血清、计划生育用药及中成药部分等）。有些药物由于受当地条件限制而无法使用或基层医生暂时还不能掌握其应用知识的药物，切记不可随意使用，必要时应请教有经验的上级医生或将患者及时转到上级医院进行治疗。
本书在编写过程中，陈再兴和闵冬雨在最后的校对和编辑方面做了大量工作，在此表示感谢。限于编者的认识水平，本书在内容和编排上难免存在错误和疏漏之处，欢迎读者特别是基层的医务工作者提出批评和宝贵意见，以便再版时进一步改进。
