Author Description

李明博士（Dr-MingLi），李明博士现为俄亥俄大学体育管理系教授，系主任。他曾在南佐治亚大学休闲与运动管理系担任副教授。李明教授所授课程有体育管理、体育经济学、体育财务管理、设施及赛事管理、国际运动管理、体育组织管理、研究法以及休闲体育管理应用统计学。
李明在中国广州体育学院体育系获学士学位，中国杭州大学获硕士学位，美国堪萨斯大学获运动管理教育博士学位。他曾任北美体育管理学会理事、全美体育学会下属体育管理委员会主任委员、国际体育健康休闲以及舞蹈协会下属的国际体育管理委员会主任、《体育管理学刊》以及《国际体育管理学刊》的编辑委员。
1996年，李明博士曾担任亚特兰大奥运使节一职。目前李明教授已在美国（（体育营销季刊》、《体育管理学刊》、《国际体育管理学刊》等著名刊物上发表20多篇论文；他也曾在美国以及国际相关研讨会上发表诸多演讲。

如何鉴别诊断马尾神经鞘膜瘤与腰椎间盘突出症？
马尾神经鞘膜瘤附着于神经根，位于腰骶段，圆锥内5个节段的高度等于一个椎骨的高度。此部位的脊膜内脊髓外的肿瘤原始症状亦是疼痛，感到腰部疼痛，并出现向下肢放射痛，当肿瘤渐增大时表现感觉丧失并肌肉无力，跟腱反射、肛门反射消失，以及出现长束症状和体征。肿瘤侵及圆椎，最常见的症状为膀胱、直肠功能障碍。腰2椎管平面以下肿瘤，常可累及马尾神经根，下腰痛为主要症状，次之为根性痛。由于此椎管容积较大，除肿瘤生长很大压迫腰骶神经根以外，出现鞍区感觉障碍，肛门括约肌张力低，肛门反射消失。此时常需与腰椎间盘突出症鉴别，因为腰椎问盘突出症95％发生于腰4～腰5、腰5～骶1椎间隙。腰椎间盘突出症的临床表现易与马尾神经肿瘤相混淆。出现下肢根性疼痛或膀胱、直肠括约肌功能障碍，中央型腰椎问盘突出症一般亦发生腰4～腰5、腰5--骶1之间，压迫马尾神经，出现腰脊痛、坐骨神经痛、双侧大腿、小腿、足底及会阴区麻木。膀胱及直肠括约肌无力或麻痹，跟腱反射、肛门反射消失。腰椎间盘突出症引起的神经症状往往是多在较严重的腰痛、下肢痛后突然发生，部分病例在原先诊断为腰椎间盘突出症，在治疗过程中特别是在牵引或手法治疗后突然瘫痪。x线表现腰间盘突出症时可出现腰椎侧弯，腰椎后凸，椎间盘变窄或前窄后宽，椎体出现骨赘，有滑脱现象，马尾神经或脊髓和椎部肿瘤大多数不正常腰椎平片。脊髓造影时椎间盘突出症侧位片示：病椎间隙处有椎间盘突出所致的造影剂弧形压迹正位片示偏向一侧的造影剂部分梗阻。在中央型腰椎间盘突出则示完全性梳齿状硬脊膜外梗阻影。马尾神经鞘瘤造影出现典型的杯口状梗阻影。
