（三）第三个新亮点：中共中央决定从新经济学理论出发明确什么是公共产品，什么是公益性
《中共中央国务院关于深化医药卫生体制改革的意见》于4月6日正式发布。意见描绘了中国到2020年的医疗改革方向和框架，确立了“把基本医疗卫生制度作为公共产品向全民提供”的改革基本原则，明确了政府在提供公共卫生和基本医疗服务中的主导地位，引发了社会的广泛关注。
把基本医疗卫生制度作为公共产品向全民提供，这在新中国历史上是第一次。基本医疗卫生服务成为公共产品，也意味着成为人民基本权利的一部分，要被全民公平享有，制度一经确立，就具有长期稳定的特征。由政府主导的回归公益性的基本医疗卫生制度，不仅符合医疗卫生事业的基本属性，也是很多国家的通行做法。
新医改方案不仅为我们提供了思考医改问题的平台，同时也为我们思考政府应该提供什么样的公共产品，有哪些公共产品应由政府提供而没有提供，哪些产品又应由市场提供等问题提供了空间。
社会产品分为公共产品和私人产品，纯粹的公共产品是这样的产品，即每个人消费这种产品不会导致别人对该种产品消费的减少，公共产品具有效用的不可分割性、消费的非竞争性和受益的非排他性。这些特点决定了公共产品应主要由政府提供，而私人产品则应交由市场，比如，人们购买一件衣服，进出一次餐饮场所，政府有监管市场的职责，但没有控制市场的权力。但是，私人产品也有外部性，有外部作用的私人产品就不是纯粹的私人产品，如果私人产品的外部性作用是有益的，那么这些具有有益的外部作用的私人产品就是公益性产品或公益性服务；如果有些私人产品的外部性作用是有害的，那么这些具有有害的外部作用的私人产品就是公害性产品或公害性服务。
中共中央决定把基本医疗卫生制度作为公共产品向全民提供，不等于把医院提供的基本医疗服务作为公共产品向全民提供。仍然是把基本医疗服务作为有外部性作用的私人产品和服务向消费者提供。如果医院提供的私人产品基本医疗服务的外部作用是公益性的，政府就给予合理补偿；如果医院提供的私人产品基本医疗服务的外部作用是公害性的。

2009年是新中国成立60周年，也是中国卫生事业发展史上的重要年份。全国人民，特别是医疗卫生系统期盼已久的《中共中央、国务院关于深化医药卫生体制改革的意见》（以下简称《意见》）和《国务院关于印发医药卫生体制改革近期重点实施方案（2009～2011年）的通知》（以下简称《实施方案》）已经发布，这是我国政治生活中的大事，更是医疗卫生系统的头等大事、重中之重，业内人士莫不欢欣鼓舞。
人们普遍关注的大问题，如深化医药卫生体制改革的重要性、指导思想、基本原则和总目标、完善医药卫生四大体系、建立覆盖城乡居民的基本医疗卫生制度以及五项重点改革等都已明确，并被广泛关注。全国医疗卫生系统在党和政府的领导下，正在加强组织领导、财力保障和宣传引导，鼓励各地积极进行试点。
本卷“医疗卫生绿皮书”——《中国医疗卫生发展报告No.5（2009）》遵照《意见》和《实施方案》精神，把加强宣传引导，坚持正确舆论导向作为义不容辞的任务。本书编委会和课题组共同努力，在深入解读新医改方案的基础上，努力总结、宣传医改的经验，解答群众关心的问题，为深化医改营造良好的社会和舆论环境。
“总报告”部分解读医改“在政治上务虚的新亮点”和“在经济上务实的新亮点”。“改革篇”中介绍了医改的背景和过程，以及在医改辩论中的思想交锋，力求使读者从不同侧面理解新医改方案是在充分发挥国务院十几个相关部委的作用，广泛征求业内外人士意见的基础上形成的，这个方案来之不易。同时强调新医改方案突出公益性，突出“政府主导与发挥市场机制作用相结合”的特点，“四梁八柱”有利于撑起基本医疗保障。本篇也有重点地介绍了一些省市的医改试点经验。“发展篇”包含《中国卫生费用核算研究大事记（1981～2008年）》以及《2007年中国卫生总费用分析与预测》、《中国公共卫生的历史沿革、现状与未来挑战》、《PDP在医院绩效管理中的应用》等文章。“经验篇”介绍了上海和北京的实践经验。“借鉴篇”对国内外病人安全管理进行了综述。
