Book Abstract

2.辅助检查
（1）尿常规化验尿呈酸性，有多数白细胞、红细胞和少量蛋白。
（2）尿细菌学检查晨尿沉渣涂片找结核杆菌，约50%～70%的病例可找到抗酸杆菌，应连查3次，必要时重复检查。
（3）结核杆菌培养阳性率高达90%，这对肾结核的诊断有决定性意义，但培养时间长达4～8周。
（4）放射学检查泌尿系统平片可见患肾局灶或钙化，应与结石鉴别；静脉尿路造影可见典型的肾盏、肾盂虫蚀样破坏，或棉桃样空洞阴影，同时可以了解分侧肾功能、病变程度及范围。严重者患肾不显影应行逆行肾盂造影，可显示肾脏破坏情况。
（5）膀胱镜检查可见膀胱三角区及患侧输尿管周围充血水肿及浅黄色结核结节或溃疡与肉芽肿。必要时取活组织检查明确诊断，患侧输尿管口可呈“洞穴”状，膀胱挛缩或急性期炎症时忌做此项检查。
（6）B超和CT检查B超可显示肾结构紊乱、脓腔和对侧肾积水；CT对晚期病变的诊断优于静脉尿路造影，可显示肾皮质空洞、钙化及输尿管管壁增厚等。
（7）核素肾动态扫描可了解分肾功能和上尿路排泄情况。
【治疗原则】
肾结核是全身结核的一部分，要注意营养、休息、避免劳累。
1.药物治疗
适合病变较轻，范围局限的早期肾结核。抗结核治疗开始前必须明确诊断，对于确诊为肾结核的患者，无论其病变程度如何及是否需行手术治疗，均需按规定进行抗结核的药物治疗。采用联合用药，短期化疗。异烟肼（雷米封）300mg/d，顿服；利福平450～600mg/d，顿服；吡嗪酰胺25mg/（kg·d）（每日不超过2g），分3次口服。吡嗪酰胺仅用于头2个月，以后改为乙胺丁醇1g/d，持续服用6个月以上或根据病情适当延长。治疗期间应定期检查肝肾功能。对耐药菌株未确立的严重病例，近来多主张加用链霉素，但需注意其对听神经的损害。

在《临床医疗护理常规》（2002年版）的基础上，我们组织了北京医师协会泌尿外科专科医师分会19位专家，编写了泌尿外科常见疾病的诊疗常规。本书力求内容准确、文字简练、条理清晰、简便实用，以同整个系列丛书的风格保持一致。
由于时间仓促及编写者的经验及水平有限，本书难免存在疏漏之处，希望广大泌尿外科同仁及广大读者提出宝贵意见，使之不断完善。
