在腕管综合征中，正中神经的周边部分在屈肌支持带（腕横韧带）下的腕管中受到压迫。原因包括类风湿性关节炎、其他关节疾病或是导致腕管变窄的代谢失调。典型症状有疼痛、感觉异常和肿胀，因所处腕关节的位置不同而异。在前三指和第四指桡骨的一半，会出现典型症状——感觉丧失。肌电图显示穿越腕管的神经传导会减慢。疼痛区域范围较广而不是只分布在正中神经的位置，不仅在受压点感觉得到，在其周围邻近的部位也能感觉得到。随着腕管综合征的发展，会导致拇指外展无力和侧鱼际肌的萎缩（外展拇短肌）。通过向腕管内注射可的松和局部麻醉剂的混合药症状可以临时得以缓解；如果症状缓解，同时也确定了诊断，因为颈源性疼痛用这种方法是不能减轻的。症状相似的其他的末梢神经损伤，例如尺神经压迫（肘管综合征），也可以同颈神经根综合征区分开来了。
因为颈椎综合征会引起肩、颈疼痛，因此鉴别诊断也必须包括肩部的原发性障碍的诊断，最重要的是产生肩峰下撞击综合征的肩峰下关节附近区的退行性变。病人主诉的冈上肌肌腱和冈下肌肌腱受到的损伤被称为岗上肌或岗下肌综合征，或者叫肩峰下撞击综合征。颈椎综合征和肩周炎的主要症状对比见表9.7。正如前面已经提到的那样，肩周炎可以发生在已患有颈椎病的患者身上，然后独立发展成病人的主要病症。在这种情况下，颈椎活动受限、颈背肌肉紧张、肩部僵硬等症状会同时出现，须平行治疗。
Paucoost综合征包括上肢疼痛、感觉异常，并伴有霍纳综合征和同侧躯干上四分之一区域出汗不足的症状。发病机制通常是肺尖肿瘤穿过最近的肺尖胸膜侵犯臂神经丛内侧束。疼痛的分布区很容易与C7和C8综合征的症状混淆。
带状疱疹可能在颈椎节段发作，但是较为少见，产生剧烈的颈肩痛。它通常不易被发现，往往被作为颈间盘综合征来治疗直到皮肤上出现水泡。一般说来，病史、体检和实验室检查对于带状疱疹脊神经根炎和Borrelia脊神经根炎的诊断就可以提供足够的依据了（由蜱叮咬引起）。
