Book Abstract

4.处方设计设计栓剂处方时应当考虑下述问题。
（1）用药的目的与吸收特性：首先应明确该栓剂是局部作用还是全身作用；给药部位是肛门还是阴道，或是其他部位；作用时间是快速显效还是缓慢释放达到长效目的。除局部作用的栓剂外，全身作用栓剂剂量的确定是首先应考虑的问题。一般认为，欲起全身作用的栓剂，其直肠给药的剂量是口服剂量的0.5 ~2倍或更多，确切的剂量宜通过直肠给药后药物体内的生物药剂学研究来确定。一个药物制成某一剂型是否有价值，主要看它是否突出了这个剂型的特点，但并不绝对。如阿司匹林的可可豆脂栓用药4h后的血药浓度只是口服给药的38%，但因阿司匹林口服给药的副作用大，故其栓剂剂型仍被使用。
（2）基质的选择与用量的确定：在药物、给药途径、作用特点确定后，处方设计的实质是基质的选择与用量的确定。一般说来，局部用药应选熔化、液化慢，释放慢、使药物不被吸收的基质；全身用药则宜选能加速释放与吸收的基质。若用油脂性基质还需注意加防腐剂、抗氧剂等附加剂；为促进吸收还可加吸收促进剂如表面活性剂等。
5.制备方法
（1）普通栓剂：制备方法有3种，即搓捏法、冷压法及热熔法，可按基质的不同和制备数量而选择。用脂肪性基质制栓，可采用上述的任何一种方法，用水溶性基质制栓多采用热熔法。药物与基质的混合可采用下述方法进行：①油溶性药物，如樟脑、冰片等，可直接混入基质使其溶解。但是，如果加入的量较大，能降低基质的熔点或使栓剂过软，此时可加适量石蜡、蜂蜡或其他混合基质，以调节其熔距。②不溶于油脂而溶于水的药物，如生物碱盐、浸膏等，可加少量的水制成浓溶液，用适量羊毛脂吸收后再与基质混合。③不溶于油脂、水或甘油的药物，可先制成细粉，再与基质混合均匀。
搓捏法：取药物的细粉置乳钵中，加约等量的基质研匀后，缓缓加入剩余的基质，制成均匀的可塑性团块，必要时可加适量的植物油或羊毛脂，以增加可塑性；再于瓷板上用手隔纸搓揉，轻轻加压转磙成圆柱体；然后按需要分割成若干等分，搓捏而成适当的形状。这种方法适用于制备小量栓剂。
冷压法：此法系利用制栓机制备。先将药物与基质置于冷容器内，混合均匀，然后置于制栓机的圆筒内，通过模型压成一定形状。机压模型成型，较美观一致，适用于大量生产。
热熔法：将计算量的基质锉末在水浴或蒸气浴上加热熔化（注意勿使温度过高），然后将不同的药物加入研磨混合，使药物均匀分散于基质中，然后倾人已冷却并涂有润滑剂的栓模中，至稍微溢出模口为度，冷却，俟完全凝固后用刀削去溢出部分。开启模具，推出栓剂，晾干，包装即得。注意：一般在基质熔融达2/3时应停止加热，适当搅拌，这样可使其全部熔融而避免过热。熔融的混合物在注模时应迅速，并一次注完，以免发生液流或液层凝固。
栓剂模孔内所涂用的润滑剂通常有两类：①脂肪性基质的栓剂，常用软肥皂、甘油各1份及95%乙醇5份所制得的醇溶液为润滑剂。②水溶性或亲水性基质的栓剂，则用油性液体为润滑剂，如液状石蜡、植物油等。
