Book Abstract

应用抗生素治疗感染性疾病，无法顾及人体自身的驱邪能力，仅是极端地杀灭全部病原微生物，当人体免疫功能低下时，抗生素治疗则难以奏效。
许多研究结果表明，清热解毒药，在体外均未被证明有较强的抗病原体作用，也没证明药物在体内转化为某种较强的抗病原体物质。有人用临床疗效较好的穿心莲内酯类物质，给家兔灌服，于不同时间测定家兔血清中是否出现抗菌物质，结果在很大剂量下，所得结果仍为阴性。实际上清热解毒药的抗病原体作用与其临床疗效常不一致，如穿心莲水溶性黄酮部分在体外有较强的抗菌作用，但临床疗效不好。其内酯部分体外无抗菌作用，临床却有效。可见仅以抗菌作用为指标来衡量清热解毒药的作用是不够的，它们必然通过另外一种作用途径来治愈疾病，这也体现了中医治疗的整体观和驱邪而不伤正的思想。
具体表现在三个方面：
1.抗毒素作用：清热解毒药在抑杀致病微生物同时常又能防止细菌毒素对人体的伤害，改善全身症状，做到菌毒并治，标本兼顾。具体体现为一方面可以降解内毒素，拮抗外毒素。比如清热药中金银花、连翘、蒲公英、败酱草等有抗内毒素的作用，前两味药作用最强，并发现龙胆泻肝汤能明显降低实验性内毒素血症的动物血浆中内毒素的含量；清热药复方“清解灵”（白头翁、蒲公英、败酱草、大黄、甘草）、“热毒清”（金银花、蒲公英、大青叶、鱼腥草）等能直接破坏和降解内毒素的形态结构，使其失去毒性。另一方面，可以降低细菌的毒力。如射干等有抗透明质酸酶的作用，故可阻止细菌毒素在结缔组织中的扩散，间接降低细菌的毒力；又如黄芩、知母、牡丹皮及黄连解毒汤在低于抑菌浓度时就能抑制凝固酶的形成，有利于细菌在体内的消灭。
2.双向调节增强免疫功能：一方面，可以促进自身免疫功能。如蒲公英、金银花、生地、牡丹皮、鱼腥草、野菊花、穿心莲等，增加白细胞素，促进白细胞和单核巨噬细胞的吞噬功能。又如，马齿苋能显著提高正常家兔淋巴细胞和PHA诱导的淋巴细胞的增殖能力，通过提高细胞免疫功能，从而调节机体的免疫功能。又如，白花蛇舌草的体外抗菌力很弱，药理实验显示，只对金黄色葡萄球菌、痢疾杆菌有微弱的抑菌作用，但在体内却能刺激网状内皮系统增生和增强吞噬细胞活力等，提高人体自身免疫力。再如山豆根、黄连、黄芩、蒲公英、银花等可提高淋巴细胞的转化率和玫瑰花结反应，呈现促进细胞免疫作用。
