二、伤寒与温病争论的启示

自汉代张仲景以来，《伤寒论》基本统治外感热病。至金元时期刘完素创寒凉派，便拉开温病与伤寒争论的序幕。伤寒学派与温病学派争论的焦点在于：伤寒是否包括温病，伤寒方是否可以治疗温病，这个争论甚至持续到新中国成立后。

祝味菊正式学习中医始于成都，到上海后，“以治川人之法”而悬壶，属伤寒学派。一般认为，叶、吴温病学说盛行于大江南北，而川医则多持伤寒之论，祝氏亦不例外。1931年，祝味菊出版《祝氏医学丛书》10种，其书目为：《病理发挥》《诊断提纲》《伤寒新义》《伤寒方解》《金匮新义》《金匮方解》《内经精华新释》《药物经验谈》《外科证治一得》《医案录粹》。从中可见祝氏学术的重点在于《伤寒论》和《金匮要略》。此外，其名著《伤寒质难》，即以伤寒论之方治西医之肠伤寒病。他认为：“仲景伤寒，历代所宗，虽有议者，未敢僭越。至吴又可而其说大变，有清叶（桂）吴（瑭）倡温热之论，伤寒捐弃，时方风行，而卒有陆（九芝）戴（北山）之流辟之，然亦未能自外于温热。”

祝味菊反对温病学者将外邪分为寒邪、温邪，温病与伤寒完全不同的说法。“寒温之辩，聚讼数百年矣，其主要之区别，在证候不在原因，然辩之者，必日其因有别，其治有殊”，祝氏看来，温病就是伤寒中症状偏于热者。“温热病者，病之偏于热也，即病者反应之偏于亢者盛也，非实有温热之邪也，亢盛之反应，即五段中之阳明也，伤寒可以包括温热，而温热仅占伤寒之一格而已”。“叶氏适应环境，而著述温热之篇，所以别于适用温药之伤寒，非另有温热之邪也。夫伤寒温病，二而一，一而二也。言刺激则有伤寒之邪，言反应，则有亢盛之体。邪正相搏其抵抗之趋势，倾向太过者，即是温热之病，温热者，病之偏于热也，热者人体反应之偏于亢进也。”在祝氏看来，叶天土的功劳，并非在于他创立了温病学说，而在于对一部分适用凉药的伤寒病，补充了治法，因为温病与伤寒本来就是一回事，伤寒可以包括温病。显然，长期经历中西医汇通考炼的祝味菊，始终自命为伤寒学派中人，这也是祝氏擅长用温药，号称“祝附子”的原因。

祝味菊对温病派的批评，并不仅仅在于理论上，而主要在于治疗上。陆九芝《世补斋医书》十二卷《续苏谈防其说》尖锐批评苏派医生：“按日开方所防皆险……其明日必至之状，皆其昨日所防及……病家不咎其手法之疏，转赞其眼力之高。”病家和医家都共认“此病本有是天然之节奏者”，“而不知病本可以不若是也”（《姜春华论医集》）。祝昧菊对此深有同感。1927年祝氏从四川来上海，侍诊于某温病名家之门，“凡三月，深佩其机巧莫测，料变若神，然病者往往由轻而重，而死，医者逐步料到，而终不能挽其死，由辛凉解表，甘淡驱湿，而至透热转气，清营散血，由宣化湿浊，滋阴清热，而至涤痰开窍，平肝熄风，医者逐步做到，而终不能弭变”。

为什么温病派医家能“熟悉疾病之趋势而不能改变其趋势”呢？祝氏认为根本的原因是他们排斥伤寒法，不敢用温药。在理论上以温病与伤寒对立，在病因上以温邪与寒邪对立，就必然在治法上以温法与寒法对立。他们“以为温热之邪，于法宜用寒凉也”，“处方虽有前后缓急之法，而赏用清凉，其揆一也”。温病派医家即便对于“湿胜阳微之体”，明知过用寒凉药会“成功反弃”，仍认为“法当清凉”。叶天士自己也说：“热病救阴犹易，通阳最难。”喻嘉言、柳宝诒等都讲到温病出现阳虚者，要用温药。而伤寒学派对此有丰富的治法，遗憾的是温病派因为自身理论上的缺陷而排斥温法。所以祝味菊说：“叶氏之后，吴鞠通、王孟英辈推波助澜，以为叶氏之温热，足以颉颃仲景之伤寒，疵谬矛盾，不胜枚举。”
