Book Abstract

侧副韧带损伤（collateral ligament injury）
〔临床表现〕-般都有明显外伤史。受伤时可听到韧带断裂的响声，很快便因剧烈疼痛而不能继续运动或工作，膝部伤侧局部剧痛、肿胀、有时有淤斑，膝关节不能完全伸直。韧带损伤处压痛明显，内侧副韧带损伤时，压痛点常在股骨内上髁或胫骨内髁的下缘处；外侧副韧带损伤时，压痛点在股骨外上髁或腓骨小头处。
〔病因〕为膝内翻（外侧副韧带损伤）或外翻（内侧副韧带损伤）强力所致。
〔体征〕关节肿胀、积血，浮髌试验阳性。侧压试验（内外翻试验）：膝关节伸直，检查者一手握住伤肢踝部，另一手掌的大鱼际顶住膝上部的内侧或外侧，强力内收或外展小腿，如内侧副韧带部分损伤，外展时因牵扯损伤的韧带引起疼痛；如完全断裂，则有异常外展活动度。反之，如外侧副韧带部分损伤，内收时因牵扯损伤的韧带引起疼痛；如完全断裂，则有异常的内收活动度。
〔辅助检查〕
1.X线检查：在局麻下，伸直膝关节，按上述检查方法，强力使膝内收或外展，拍正位X线片。如侧副韧带完全断裂，则伤侧关节间隙增宽。
2.磁共振：用于检查有无合并半月板及前后交叉韧带损伤，还可发现意料不到的韧带结构损伤与隐藏的骨折线。
〔诊断〕临床症状+体检+辅助检查。
【鉴别诊断】急性损伤性滑膜炎；半月板损伤；交叉韧带损伤；膝关节周围骨折。
〔治疗〕
1.新鲜侧副韧带损伤
（1）部分断裂：将膝置于150°-160°屈曲位，用长腿管型石膏固定（不包括足踝部），一周后可带石膏下地行走，4~6周后去除固定，练习膝关节屈伸活动，注意锻炼股四头肌。
（2）完全断裂：应急症手术修复断裂的韧带，术后用长腿管型石膏固定6周。如合并有十字韧带损伤，应先修复十字韧带，然后修复侧副韧带；如合并半月板损伤，应先切除损伤的半月板，然后修损伤的韧带。
2.陈旧性侧副韧带断裂：应加强股四头肌锻炼，以增强膝关节的稳定性，如膝关节很不稳定，可用邻近部位肌腱作韧带重建术。
交叉韧带损伤（cruciate ligament injury）
〔临床表现〕膝软无力，受伤时可听到响声或有撕裂感，关节迅速肿胀。大约70%的急性膝关节创伤出血伴有交叉韧带损伤。肌肉保护性痉挛、疼痛，多不能立即站立承重。由于撕裂的交叉韧带嵌入关节间隙，可发生交锁。
〔病因〕多有外伤史，多为非接触损伤。常见机制为膝前直接受力、奔跑时突然停止或者足部卡住、膝部受剪切力或者滑雪时摔倒。如膝屈曲90°，小腿后方受到打击也能单独损伤前交叉韧带。
〔体征〕关节肿胀、积m，浮髌试验阳性，抽屉试验，轴移试验阳性。
〔辅助检查〕
1.X线检查：①正位X线片上胫骨平台外侧有撕脱骨片时，多为前交叉韧带已断裂，此现象为Segond阳性；②胫骨髁间棘撕脱骨折说明前交叉韧带下止点撕脱断裂；③90度前后抽屉试验下，如果胫骨前后移动超过Smm，说明前或后交叉韧带断裂。
2.磁共振及关节镜：对交叉韧带损伤有诊断意义，可检查有无合并半月板损伤，还可发现意料不到的韧带结构损伤与隐藏的骨折线。
〔诊断〕临床症状+体检+辅助检查。
〔治疗〕
1.前交叉韧带损伤：凡不满2周的前交叉韧带断裂，应争取手术缝合。如果在韧带体部断裂，最好再移植一根肌腱以增强交叉韧带的稳定性。一般选用髌韧带的中1/3作为移植材料。对部分断裂者，可以缝合断裂部分，再石膏制动
