Book Abstract

3.治疗
（1）有活动性出血者，应结扎止血，清除血块后，缝合伤口，加压包扎。对于皮下脂肪的血肿（未穿透筋膜者），清除血块，止血后，可开放伤口，换药待肉芽生长，蝶形胶布牵拉对合切口，一般5～7天可愈合。
（2）深部血肿（陈旧性血肿）可在B超指引下穿刺抽吸，应注意无菌操作，一次抽吸一般不超过20ml，加压包扎，1-2天后，可行第2次穿刺抽吸。一般抽吸2次后，可以保守治疗成功。对于有活动性出血、血肿，应手术治疗，结扎止血。
（3）不论浅或深部出血，都应给予抗生素预防感染或控制感染，短期停用抗凝药物。
4.典型病例
（1）患者，28岁，于1997年12月10日因胎儿宫内窘迫、妊娠高血压综合征在硬脊膜外麻醉下行子宫下段剖宫产术（横切口）。术前Hb91g／L，出、凝血时间正常。手术顺利，术中出血量约150ml。术后7天拆线，切口I期愈合。拆线后3天左下腹痛加重，B超检查见左下腹有一11.3cm×9.6cm大小囊性包块。妇科检查：子宫前方、腹壁下可触及胎头大小包块。考虑腹壁血肿，即在局麻下行血肿清除术。沿原切口切开皮肤及皮下组织，见筋膜下有大量凝血块和暗红色黏稠不凝血，约800ml，自脐至耻骨联合间形成巨大腔隙，未见活动性出血。清除积血、冲洗创腔、置橡皮条引流后用7号丝线全层间断缝合腹壁。术后7天拆线，痊愈出院。
腹壁血肿是腹部手术较常见的并发症，但巨大血肿临床上少见。剖宫产术后腹壁积血达800ml，实属罕见。本例血肿形成的原因有：①开腹时，过分牵拉分离腹直肌，损伤了腹直肌及下方的腹壁下动、静脉；②关腹时未严密缝扎腹膜外和肌肉的出血。本例提示，术后应严密观察切口有无压痛及肿块，如有异常应及时行B超检查或穿刺，一旦确诊应及早手术。术中如有活动性出血应缝扎，缝合腹壁时应放置橡皮引流条，术后加压包扎，并配合理疗，大多能I期愈合。
