三、对侧冠脉造影
慢性完全闭塞病变介入治疗之前术者应仔细分析病史和观察冠脉造影，因为这有助于术者选择最佳的介入治疗方法、预测可能出现的问题及选择相应的处理措施。在慢性完全闭塞病变介入治疗中，通过对非闭塞血管造影，利用其侧支血管灌注闭塞远端血管观察整个闭塞血管段非常重要。尽管我们不需要在所有的慢性闭塞病变的介入治疗中进行对侧冠脉造影，但是该方法对判断闭塞远端血管形态、闭塞段长度、闭塞血管的大致走向和导引钢丝前进方向正确与否起着非常重要的作用（图4-2）。
在进行对侧冠脉造影时，通常采用4-6F造影导管，为了使闭塞远端血管充分显影，必要时应适当延长对侧造影时间。通过对闭塞近远端血管的观察，可以大概了解闭塞血管段的长度。当闭塞远端血管的直径比近端大或者闭塞远端血管成锥形时，有可能提示该病变比较硬，闭塞远端的纤维帽比较厚。对于呈锥形的闭塞远端血管，导引钢丝通过该处时必须十分小心，因为一旦在该处形成假腔，导引钢丝将很难进入血管真腔（图4-3，图4-4）。治疗过程中，当术者不能确定导引钢丝的走行方向是否正确时，也需要进行对侧冠脉造影，尤其是当导引钢丝行进在血管弯曲部分时。

Introduction

冠状动脉慢性完全闭塞病变（chronic total occlusion，CTO）约占全部冠脉造影检出病变的1／5，但接受经皮冠状动脉介入治疗（percutaneous coronary intervention，PCI）者少于8％，约占全部PCI病例的10％～20％。与非闭塞病变相比，CT0病变介入治疗的手术成功率低，并发症、再狭窄和再闭塞发生率高。尽管不断涌现新器械和新技术，CTO手术成功率有了很大的提高，但CTO病变仍然被认为是目前PCI治疗领域最大的挑战。近几年来，我国接受PCI的患者例数不断增加，从事冠脉介入治疗的医生数量也呈快速上升趋势，但各地区之间介入治疗水平参差不齐，尤其是CTO治疗水平有很大的差异，亟待规范。
