1 干细胞的临床应用潜能：希望与否
1.1 绪论
当前，人们对干细胞研究的热情和争议起始于2个关键的突破：1997年Ian Wilmut，Keith Campbell及其合作者成功培育出克隆羊“Dolly”；1998年James Thomson成功建立了人的胚胎干细胞（ESC）系。这些新技术的出现无疑为组织工程和器官移植开辟了新的途径E引。在生物医学的研究历史上从未出现某一科学的发现能在世界范围产生如此巨大的冲击。使人体再生甚至更换缺陷的器官或组织是人类百年的梦想。干细胞具有能分化出机体所有特定类型的细胞和组织的潜能，因此可以和人类很久以来一直追求的“青春泉”相媲美。最近使用成体和胚胎干细胞作为起始细胞群的研究，让我们推测使用这样的“原材料”或许可以为再生医学生产各种替代的部分。许多年龄相关的退行性紊乱疾病例如心脏病、帕金森病、糖尿病和中风等很有希望在未来某一天通过干细胞治疗治愈。
1.2什么是干细胞
所有的生命形式都起始于一个干细胞，可以定义为具有自我更新和产生祖细胞以及生物体中不同类型细胞的双重能力的一种细胞。例如，在人生命的起始阶段，一个受精卵（合子）变成2个细胞，2个再变成4个细胞。在这些早期阶段，每个细胞或许仍然具有全能性，也就是说这些细胞每一个都可以单独发育成一个生物个体。在5～7天内，约40个细胞形成内细胞团，外层的细胞将来发育成胎盘。这个时期的内细胞团的每一个细胞都具有产生包括生殖细胞在内的所有组织类型和器官的能力，即这些细胞都具有多能性（图1.1）。内细胞团的细胞最终将产生组成人体的10个细胞，其中有200多种分化的不同类型细胞。
