Book Abstract

（3）硬膜外阻滞：
1）利多卡因混合1：200 000肾上腺素。
2）罗哌卡因0.2％～0.5％。
o.5％布比卡因——起效最慢、持续时间最长。
3％2－氯普鲁卡因——起效最快和持续时间最短。
硬膜外阿片类药与上述局部麻醉药一起给药（注意：硬膜外阿片类药用量是脊椎麻醉阿片类药剂量的5～10倍）：①芬太尼50～1150ug；②舒芬太尼10～20mg；③吗啡5mg；④哌替啶50～100mg。硬膜外吗啡3～5mg联合芬太尼25～50tzg能获得快速起效和长持续时间的镇痛作用。
上腺素可添加于局部麻醉药中以减少全身吸收，延长麻醉持续时间和增强感觉阻滞强度，也能增强运动阻滞强度。
急诊
脊椎麻醉：
①3％2－氯普鲁卡因20～30mg；
②利多卡因混合1：200 000肾上腺素15～20mg。交感阻滞可降低全身血管阻力及增加静脉容积，产生低血压。这些改变导致外周血滞留，前负荷降低。在脊椎麻醉或硬膜外麻醉前，预补液、左侧子宫斜楔位、肌注肾上腺素（阻滞前约l0分钟给药），可预防性避免低血压的发生。常规监测孕妇血压、氧饱和度和心电图。必要时静脉注射5mg肾上腺素。母亲或胎儿在危急时发生意外并发症
·急诊剖宫产术进行气管内全麻。
·100％纯氧深呼吸4～5次。
·压迫环状软骨行快速诱导
·丙酚+硫喷妥钠+氯胺酮
·插管前，使用氯琥珀胆碱无需预给剂量。
·可能不需要或仅需要最少量的非去极化肌松药。
·0.5 MAC和100％FiO2。
@脐带钳夹后，使用止痛药和咪哒唑仑。
