孙升云，医学博士，副教授，副主任医师，广州暨南大学附属第一医院中医科主任。广东省省情咨询与对策研究专家，广东省中医药学会理事，广东省中西医结合学会肾病专业委员会常务理事，广东省医师学会中西医结合医师分会常务委员。曾在第一军医大学、广州中医药大学、广东省中医院学习工作，先后师从著名肾脏病学家叶任高教授、全国名老中医黄春林、沈英森教授等学习，从事中西医结合临床、科研工作20余年，承担国家自然科学基金1项，省级课题5项。对中西医结合治疗肾脏疾病，如急慢性肾炎、狼疮性肾炎、肾结石、尿路感染、痛风肾、慢性肾衰竭、肾虚证有较深的体会。尤擅长于运用中西医结合疗法治疗难治性肾病综合征和慢性肾衰竭。

一、肾脏的基础知识
1.什么是肾脏？
每个正常人都有两个肾脏。形似蚕豆，但要比蚕豆大得多，一般人都没有见过自己或别人的肾脏，但多数人都见到过动物的肾脏，也就是“腰子”，其实人的肾脏和其他动物如猪羊的肾脏“长得非常相似”。肾脏为实质器官，其内部结构大体上可分为肾实质和肾盂两部分。肾单位是肾脏结构和功能的基本单位，每个肾脏约有100万～200万个肾单位，每个肾单位都由一个肾小体和一条与其相连通的肾小管组成。每个肾小体包括肾小球和肾小囊两部分，肾小球是一团毛细血管网；肾小囊有两层，均由单层上皮细胞构成，外层（壁层）与肾小管管壁相通，内层（脏层）紧贴在肾小球毛细血管壁外面，内外两层上皮之间的腔隙称为囊腔，与肾小管管腔相通。肾小管长而弯曲，分成近球小管、髓袢细段、远球小管三段，其终末部分为集合管，是尿液浓缩的主要部位。肾单位之间有血管和结缔组织支撑，称为肾间质。
肾实质可分为肾皮质和肾髓质。在肾脏的切面上，可见深红色的外层为皮质，浅红色的内层为髓质。皮质包绕髓质，并伸展进人髓质内，形成肾柱；髓质由十几个锥体构成，锥体的尖端称为肾乳头，伸入肾小盏。每个乳头有许多乳头孔，为乳头管的开口，形成筛区，肾内形成的尿液由此进入肾小盏、肾小盏呈漏斗状，每个肾小盏一般包绕1个肾乳头，也有包绕2～3个的。
