5.骨髓抑制者慎用。
6.肝、肾功能不全患者慎用。
7.有免疫缺陷的患者慎用。
8.有机会性感染病史的患者慎用。
9.治疗期间应定期（一周至少1次）监测全血细胞计数。
10.本品溶液配制时，先用2ml无菌注射用水溶解50mg本品，使浓度为25mg／ml。使用时，再用100ml或125ml生理盐水或5％葡萄糖注射液稀释。
11.操作和配置本品注射液时应谨慎。推荐使用乳胶手套和防护眼镜以防因小瓶破损或其他偶然的溢出而引起的药物接触。若皮肤或黏膜接触到本品，应立即用肥皂水和清水彻底清洗污染部位。如药液进入眼部，应使用大量清水彻底淋洗眼部。应避免吸入本品。
12.应遵守正确的操作规程。按照细胞毒性药物常规处理方法操作。溢出物或者废弃物应使用烧灼灭菌法处理。
13.用药时间的长短由治疗成功与否和药物耐受情况决定。慢性淋巴细胞白血病患者应用至达到最佳疗效（完全缓解或部分缓解，通常6个疗程）再停药。
14.男性患者用药期间及用药后6个月内，必须采取避孕措施。
15.用药过量可引起不可逆的中枢神经系统毒性，表现为迟发的失明、昏迷和死亡。还可引起骨髓抑制，造成严重的血小板减少和粒细胞减少。本品过量尚无特效拮抗剂，过量的治疗主要包括停用药物和支持治疗。
