问答题
1.先天性心脏病的分类：临床上根据左右两侧心脏及大血管之间有无分流分为三类：①左向右分流型（潜在青紫型）：正常情况下由于体循环压力高于肺循环，故平时血液从左向右分流而不出现青紫。当剧哭、屏气或任何病理情况下致使肺动脉或右心室压力增高并超过左心压力时，则可使血液自右向左分流而出现暂时性青紫，如室间隔缺损、动脉导管未闭和房间隔缺损等。②右向左分流型（青紫型）：某些原因（如右心室流出道狭窄）致使右心压力增高并超过左心，使血流经常从右向左分流时，或因大动脉起源异常，使大量静脉血液流人体循环，均可出现持续性青紫，如法洛四联症和大动脉转位。③无分流型（无青紫型）：左右两侧血液循环途径之间无异常的沟通，不产生血液的分流，故无紫绀，如肺动脉狭窄和主动脉缩窄等。
2.先天性心脏病的诊断要点：
（1）病史：①母妊娠史：注意孕期前3个月有无病毒感染史、服用影响胎儿发育的药物史及接触有害物质等。②常见症状：轻症病人可无特殊症状，多在体检时发现心脏杂音而就诊；重症患儿可有喂养困难、咳嗽、气促、汗多、声音嘶哑、活动或哭吵后出现短暂青紫或持续性青紫。有紫绀的患儿往往发育迟缓，并可有蹲踞现象。③发病年龄：3岁以前发现的心脏病以先天性畸形可能性大。婴幼儿期反复出现心力衰竭提示先天性心脏病的存在。
（2）体格检查：①一般表现：紫绀、生长发育异常和呼吸困难。心力衰竭者肝脏增大，肝颈静脉回流征阳性并可有水肿，可有其他先天性畸形。②心脏检查：望诊包括心前区隆起、异常心尖搏动；触诊包括心前区有抬举感或震颤；扣诊包括心脏大小及位置改变；听诊包括第一、二心音异常、杂音等。③周围血管征：比较上、下肢动脉搏动及血压，如股动脉搏动减弱或消失，下肢血压低于上肢，提示主动脉缩窄；脉压增宽，伴有毛细血管搏动征和股动脉枪击音提示动脉导管未闭。
（3）特殊检查：X线检查、心电图、超声心动图等。
3.房间隔缺损的临床症状、心脏体征及X线检查、心电图特点：①临床表现为发育落后，乏力，活动后心悸、咳嗽、气短，晚期肺动脉高压时有青紫。②心脏体征为胸骨左缘第2、3肋间可闻及Ⅱ一Ⅲ级收缩期吹风样杂音，传导范围小，无震颤，肺动脉瓣第二心音亢进，分裂固定。③X线示右房、右室大，肺动脉段凸出，肺野充血，可见“肺门舞蹈征”。④心电图示不完全右束支传导阻滞，右室肥大。
4.室间隔缺损的临床症状、心脏体征及X线检查、心电图特点：①临床表现为发育落后，乏力，活动后心悸、咳嗽、气短，晚期肺动脉高压时有青紫。②心脏体征为胸骨左缘第3、4肋间可闻及Ⅲ一V级粗糙全收缩期杂音，传导范围广；可触及震颤.P2亢进。③X线示左、右室大，左房可大，肺动脉段凸出，肺野充血。④心电图正常或左室或右室肥大。
5.动脉导管未闭的临床症状、心脏体征及X线检查、心电图特点：①临床表现为发育落后，乏力，活动后心悸、咳嗽、气短，晚期肺动脉高压时有青紫。②心脏体征胸骨左缘为第2肋间可闻及连续性机器样杂音，向颈部传导；可触及震颤，肺动脉瓣第二心音亢进。③X线示左室大，左房可大，肺动脉段凸出，肺野充血。④心电图示左室肥大，左房可肥大。
6.法洛四联症的临床症状、心脏体征及X线检查、心电图特点：①临床表现为发育落后，乏力青紫（吃奶、哭闹时加重），蹲踞，可有缺氧发作。②心脏体征为胸骨左缘第2、3肋间Ⅱ一Ⅳ级喷射性收缩期杂音，传导范围较广，可触及震颤，肺动脉瓣第二心音减低。③X线示右室大，心尖上翘呈靴型，肺动脉段凹陷，肺野清晰。④心电图示右室肥大。
