（2）城乡居民卫生服务需求与利用：因病治疗情况、需求未满足程度及原因，居民利用公共卫生服务情况、门急诊和住院服务的类型和水平，各种类型服务利用个人支付的费用等。
（3）城乡居民医疗保障：医疗保障系统的组成、医疗保险覆盖情况、补偿范围与补偿水平、主要保险制度的运行情况等。
（4）居民的满意度：对医疗卫生服务系统的满意度、医疗服务提供过程的满意度、对医疗保障制度的满意度。
（5）基层医疗卫生机构服务提供能力与质量：人力资源基本情况、人员服务提供能力、房屋及主要设备、收支状况、服务提供数量与质量。
（6）医务人员执业环境与满意度。
四、调查方法与对象
本次调查采用居民调查与服务提供机构调查相结合、定量与定性相结合、代表性调查与专题调查相结合的方法。调查内容主要包括家庭健康询问调查、基层医疗卫生机构调查、医务人员问卷调查和专题研究等。
（一）家庭健康询问调查
家庭健康询问调查采用入户询问的方法收集数据。由经过培训合格的调查员按调查表的项目对调查户所有成员逐一进行询问。家庭健康询问调查设调查员和调查指导员，都应是乡镇卫生院或社区卫生服务中心及以上卫生机构的卫生人员。调查员负责人户调查，调查指导员负责调查的组织、指导、检查及验收工作。
家庭健康询问调查的对象为所抽中样本住户的实际人口（凡居住并生活在一起的家庭成员和其他人，或单身居住、生活的，均作为一个住户）。家庭健康询问调查问卷主要由家庭一般情况、家庭成员健康情况、15～49岁已婚育龄妇女情况、5岁以下儿童情况、60岁及60岁以上老年人口情况、两周病伤情况、住院病人情况和农村地区外出务工人员情况等调查表组成。

天地之间，莫贵于人。2009年农历腊月二十九日温家宝总理在中央领导人团拜会上的讲话“为了人民生活得更加幸福、更有尊严”，余音绕梁。拥有健康的体魄是人民幸福生活的前提，拥有健康的国民是国家稳定和发展的前提。当前我国的发展正由侧重积累的阶段转变为重视提高人民生活水平以及相应的各种保障的阶段，医疗卫生改革被推到风口浪尖的位置亦在情理之中。

湖北省历来重视卫生服务调查工作。本次调查动员全省卫生系统近500余名工作人员，总结了第三次卫生服务调查扩大样本点的经验，在保持核心内容连续性和可比性的基础上，进一步扩大样本点，并按照国家样本点的标准为省级样本点配备专项调查经费，共覆盖16个县（市、区）9597户家庭28780人，获取近700万个原始数据，更加全面、系统、客观地掌握了湖北省城乡居民家庭人口经济学特征，居民健康状况及卫生服务需要，居民卫生服务需求及服务费用，卫生系统的可及性、反应性及居民满意度，妇幼保健情况，老年人生活及健康状况、健康行为及生活方式等需方信息。
本书是我省第四次卫生服务调查的果实结晶，凝聚着各样本县（市、区）卫生部门领导、组织人员、调查人员以及本书编写人员的心血。我相信，这些成果将为我省深化医药卫生体制改革和发展提供真实、准确的基础信息，将为我省构建社会主义和谐社会，实现全民小康提供重要的政策依据。在此，我代表湖北省卫生厅对参与此次工作的全体人员表示衷心的感谢！
