Book Abstract

谈及胸外科微创治疗，人们首先想到的是电视胸腔镜。胸腔镜的使用是20世纪末在胸外科领域发生的一次重大的技术革命，它改变了一些胸外科疾病的治疗概念，成为未来胸外科发展的方向。从广义上讲，在胸外科的所有领域，只要创伤小、痛苦轻、疗效可靠的诊断和治疗方法都应属于微创范畴。因此，在本书中，除胸腔镜的内容外，还包括了食管支架、气管支架、射频治疗和超声气管镜的内容。
外科手术是一把双刃剑，它在治病的同时也为病人带来创伤。胸腔镜微创术在我国开展已有十余年，与常规开胸手术相比，胸腔镜手术具有创伤小、术后恢复快、并发症少、术后生活质量高且符合美容要求等优点，特别适合年老体弱、心肺功能不佳的患者。概括地讲，我国胸部微创外科手术的手术技术日趋成熟，适用病种及手术适应证不断拓宽，各种新术式、新技术不断出现，但各级医院的发展还很不平衡。迄今为止，国内文献报告的胸腔镜手术种类已经多达数十种，包括自发性气胸、肺大疱切除、胸膜疾病活检、纵隔肿物切除、胸腺切除治疗重症肌无力、交感神经切断术治疗手汗症、心包开窗、肺楔形切除、肺叶切除、全肺切除、食管良性肿瘤切除、食管肌层切开治疗贲门失弛症等，随着经验的积累、手术技术的日臻完善、设备器械的不断更新和发展、手术适应证和禁忌证的不断筛选总结，手术适用范围及病种也在不断的扩大和普及。目前也存在一定误区，即使用腔镜就是微创。在单纯的胸腔镜手术中，腔镜通常起主导作用，贯穿整个手术。而对整个微创胸外科而言，腔镜在术中可能是主导的，也可能是辅助的；可能贯穿整个术程，也可能只协助某个操作。因此，腔镜只是胸外科医生的一种相对特殊的、重要的工具，而不是必需的工具；使用腔镜并非就是微创，不用腔镜并非就是大创伤。如食管癌病变仅仅限于黏膜层，使用胸腔镜手术，创伤肯定比传统的“拔脱术”要大；而对于食管下段贲门癌，用胸腔镜+腹颈联合切口的创伤是否就比左下胸一个切口的创伤更小呢？其次是切口撑开与切口长度的问题。微创外科的切口长度是相对的，“微创胸外科手术切口绝对微小”的定义是形而上学。对于切口撑开与大小的争论实际上牵涉具体情况具体解决的问题。有学者认为，既然是微创手术，就不应该撑开小切口；而也有一部分学者认为既然需要撑开小切口，则创伤就与传统开胸手术无异。这些都是极端的观点，医学行为应该从患者实际出发，而不应拘泥于纯理论的争论，只要患者能够明显从中受益，就应该首先选择。从学科发展的角度来看，胸腔镜外科发展需要解决以下几个问题：①加强培训，建立准入制度来规范手术技术；②制定、修订手术适应证并规范化；③引入临床试验和远期随访考察对比各种术式。
