足部按摩的强度，也是按摩技巧的一个重要环节。应根据受术者的体质差别、不同病症，对选取的穴位和反射区所施用的按摩强度亦应不同。
一般对严重心脏病患者的心脏反射区，肝脏病患者的肝脏反射区，以及敏感性较强的反射区，如眼、耳、三叉神经、小脑、脑垂体、胆囊、脾脏、尾骨外侧等反射区，在按摩时用力均不宜过重，只要有明显的痛感就行了。对少数痛觉特别敏感者，亦不宜用强刺激。
对那些敏感性相对较弱的反射区，如肾上腺、肾脏、输尿管、额窦、头、斜方肌、肺、结肠、直肠、腰椎、胸椎、膝、肘关节等反射区应用较大的力量进行刺激。骨骼系统的病痛，亦必须用较强的力量按摩，方能取得效果。对一些急性病的疼痛，亦可视情况加大刺激量。
