三、高血压病
(一)高血压病的定义
在静息状态下动脉收缩压和(或)舒张压增高(≥140／90毫米汞柱)，常伴有脂肪和糖代谢紊乱，以及心、脑、肾和视网膜等器官功能性或器质性改变，称为高血压病。
诊断高血压时，必须多次测量血压，至少有连续3次舒张压的平均值在90毫米汞柱(12．O千帕)或以上才能确诊为高血压。仅1次血压升高者尚不能确诊，需随访观察。
(二)高血压病的流行病学资料
高血压病是最常见的心血管病，是全球范围内的重大公共卫生问题。2004年的中国居民营养与健康现状调查结果显示，我国18岁及以上居民高血压患病率为18．8％，由此估计全国患病人数超过1．6亿。与1991年相比，患病率上升3l％，患病人数增加约7000多万。
由高血压导致的脑血管病危害极大。1998年，我国脑血管病居城市居民死亡原因的第二位，在农村居首位。全国每年死亡超过100万，存活的患者500万～600万，其中75％以上遗留不同程度的残疾，给个人、家庭和社会造成了沉重的负担。而脑卒中的主要危险因素是高血压。同时，血压升高还是多种疾病的导火索，会使冠心病、心力衰竭及肾疾病等的发病风险增高。由于部分高血压患者并无明显的临床症状，高血压又被称为人类健康的“无形杀手”。因此，提高对高血压病的认识，对早期预防、及时治疗有着极其重要的意义。我国各级政府对于高血压病的防治给予了极大的关注。自20世纪50年代起，卫生部及中国医学科学院曾组织了3次全国性高血压普查。60年代末，相关研究人员开始在国家的支持下开展以社区为基础的高血压病防治项目和相关疾病的监测活动。
1987年，卫生部先后成立了全国心脑血管病防治研究领导小组，并设立了办公室，制定了防治研究的10年规划。
自20世纪90年代以来，则更加重视高血压病的群体防治工作，制定了《全国心脑血管病社区人群防治1996～2010年规划》。1997年起在全国23个省、市、自治区建立了以高血压病防治为重要内容的慢性非传染性疾病综合防治示范点。
1998年，卫生部为提高广大群众对高血压病危害的认识、动员全社会都来参与高血压病预防和控制工作、普及高血压病防治知识，决定将每年的10月8日定为“全国高血压日”，在全国范围内掀起了防治高血压病宣传活动的高潮。
高血压病的定义还有不少争议，最近美国高血压学会(ASH)就提出血压升高是疾病的结果，而不是原因。他们认为高血压是一种多项原因引起的进行性心血管病综合征，导致心脏和血管功能与结构改变，最后出现血压升高。
严格地说，高血压与高血压病不是一回事。由于继发性苘血压有其病因，为了方便起见，一般将“原发性高血压”简称为“高血压病”。
四、高血压的分类
根据高血压的发病原因，将其分为原发性高血压和继发性高血压两大类。
(一)原发性高血压
原发性高血压的发病原因不明了，约占高血压病人的959／6以上，大多数病人有家族遗传史。
在遗传因素中，种族差别十分明显。美国黑种人高血压患者是白种人的2倍，高血压有明显的家族遗传倾向。年龄的差别也很明显，一般认为除非特殊情况，20～40岁的成年人一般血压不会发生变化。要变化的是在45岁以上及从出生到发育完全的青少年，称为新生儿高血压或青少年高血压。在55岁以前，男性比女性更易患高血压。但过了55岁，女性却比男性更易患高血压，但此时男性却更容易出现综合征。
(二)继发性高血压
如能查明原因，并确定系某种原因引起的高血压，就称为“继发性高血压”。继发性高血压是因全身性疾病引起的高血压，病因明确，最多见的是由肾疾病、内分泌疾病引起，其次是脑部炎症、肿瘤、外伤引起的高血压；某些药物也可升高血压，如激素、避孕药、甘草浸膏等。
肾疾病引起高血压，最常见为急、慢性肾小球肾炎。急性肾炎多见于青少年，起病急，常有链球菌感染史，临床表现有发热、血尿、水肿等；检验小便，尿中有蛋白、红细胞和管型。急性肾炎治疗不彻底可转为慢性肾小球肾炎，可反复水肿、明显贫血、血浆蛋白低，甚至出现恶心、呕吐、抽搐，尿素氮、肌酐明显升高，此为慢性肾炎并发尿毒症的表现。其他如多发性肾结石、肾囊肿、肾结核、肾动脉狭窄也可导致高血压。
内分泌性疾病引起的高血压，可见于嗜铬细胞瘤，病变主要是肾上腺髓质或交感神经节大量分泌去甲肾上腺素和肾上腺素。其次是原发性醛固酮增多症，病变主要是肾上腺皮质增生或肿瘤，致使醛固酮分泌增多、血压升高。再是库欣综合征，病变主要是肾上腺皮质肿瘤或增生，糖皮质激素分泌增多，血容量增多而引起高血压。妊娠妇女患高血压时，需注意是否为妊娠中毒症。引起高血压的少见原因，如先天性主动脉缩窄，主要是由先天性畸形所致。至于药物所致的高血压，往往停药后也就会恢复。
例如：王某因头痛去医院检查，发现血压升高超过150／100毫米汞柱，医生可先诊断他为“高血压待查”。做了多种检查无特殊疾病，而他的父母亲有明确高血压病情况，则我们可诊断王某患有“高血压病”。如B超发现肾脏有多个囊肿，那我们只能诊断他患有“多囊肾”，其高血压就称为继发性高血压。

高血压病的病因主要有：盐分和脂肪摄取过多的饮食习惯、运动锻炼不足、嗜好吸烟饮酒或是生活、工作压力过大等因素。
防止高血压病的发生关键在预防。高血压对身体造成的损害是一个慢性过程，不知不觉地进行着，而大约有60 9／6的人却不知道自己患有高血压病。因此，要让广大群众提高对高血压病的认识，了解高血压病的原因及并发症，早发现、早诊断和早治疗具有极其重要的意义。
高血压病是一种慢性顽固性疾病，治疗需要长期坚持，短时间控制好了，如果一旦出现情绪波动，或者劳累等情况，马上又会复发。因此，除个别病人外，大多数病人需终身服药，才能使血压维持在理想水平。
