Book Abstract

一、如何对新生婴儿进行日常护理？
新生儿身体的各个系统在出生后仍处于发育过程中，但他必须迅速适应子宫外的生活。在适应过程中，父母应当能够识别新生儿正常的生理特征，以便及时发现存在的问题，并采取正确的措施。
社区卫生服务中心的妇幼保健人员在新生儿期一般家访2～3次。家访的目的在于早期发现问题、早期干预，从而降低新生儿疾病发生率或减轻疾病的严重程度。访视内容主要包括询问新生儿出生情况、出生后生活状态、预防接种、喂养与护理情况等；观察居住环境及新生儿一般情况，如有无产伤、黄疸、畸形、皮肤与脐部感染等；体格检查，测量头围、体重等；视、听觉筛查；指导新生儿家长的合理喂养、日常护理等。发现有严重问题者应立即就诊。
母乳是新生儿的最佳食品，妈妈应坚持母乳喂养，按需哺乳，并学会观察乳汁分泌是否充足，新生儿吸吮是否有力。若母乳充足、新生儿哺乳后安静入睡、大小便正常，体重正常增长，母亲可有乳房胀痛感或乳汁溢出浸湿胸前衣服等现象。喂哺后竖抱，轻拍后背，右侧卧位，床头略抬高，避免溢奶引起窒息。同时，注意部分药物可通过乳汁分泌，故乳母应在医师指导下用药。如确系母乳不足或无法进行母乳喂养者，则应采取科学的人工喂养或混合喂养方法。
新生儿房问应阳光充足，通风良好，温湿度适宜。有条件者家庭室内温度保持在22℃～24℃，湿度55％～65％。冬季环境温度过低可使新生儿（特别是低出生体重儿）体温不升，影响代谢和血液循环，甚至发生新生儿寒冷损伤综合征，所以，新生儿在寒冷季节要特别注意保暖。家长要正确使用热水袋或代用品保暖，防止烫伤。夏季若环境温度过高、衣被过厚或包裹过严，可引起新生儿体温上升（捂热综合征）。因此，要随着气温的变化调节环境温度，增减衣被、包裹。
新生儿皮肤娇嫩，代谢旺盛，最好每日洗澡，水温以略高于体温为宜，可用中性的婴儿沐浴露或肥皂，先清洗眼睛，然后清洗整个面部及全身。新生儿脐带未脱落前要注意保持清洁干燥，避免尿液污染，每次洗澡后用酒精擦拭消毒。衣物和被褥要用柔软的棉布制品，衣服样式简单，易于穿脱，宽松不妨碍肢体活动。尿布也需用棉布，以浅色为宜，便于观察大小便的颜色和性状，且勤换勤洗，保持臀部清洁干燥，预防新生儿臀红。一旦发生臀红，可在每次大小便后用温开水清洗臀部，擦干后轻轻用棉签蘸些润肤油涂在臀部和会阴部。新生儿包裹不宜过紧，更不宜用带子捆绑成“蜡烛包”，应保持双下肢屈曲以利于髋关节的发育。
定时开窗通风，保持室内空气清新。新生儿有专用用具，奶瓶等用后要消毒。母亲在哺乳前应洗手和清洁乳头。尽量减少亲友探视和亲吻新生儿的次数，家人患感冒时须戴上口罩接触新生儿。按时接种卡介苗和乙肝疫苗。
新生儿的视、听、触觉已初步发展，父母应多拥抱和抚摸新生儿。按摩可加快新生儿的新陈代谢、降低肌肉紧张度、提高免疫力、促进对食物的消化吸收，因此，父母可在科学的方法指导下经常为宝宝“抚触”，增加父母与新生儿的情感交流，这有利于宝宝的健康成长和智力发育。另一方面，父母经常对着新生儿说话和唱歌也可促进宝宝的智力发展。
