Book Abstract

三、肱骨近端骨折
肱骨近端骨折多发生于老年女性，常因摔倒时用手撑地，暴力向近端传导致伤，青壮年在车祸等高能量暴力损伤中也可发生，且骨折更严重，往往有其他伴发损伤。根据骨折的部位、骨折块相对于肱骨头的位置以及是否合并肩关节脱位等，可分为一部分骨折、两部分骨折（或骨折脱位）、三部分骨折（或骨折脱位）、四部分骨折（或骨折脱位），肱骨头骨折根据关节面受累情况又可分为劈裂型骨折和压缩型骨折。肩关节创伤系列平片、CT扫描与三维重建有助于骨折的分型。无移位、轻微移位或闭合复位后骨折端稳定的肱骨近端骨折可采用非手术治疗，闭合复位后骨折端不稳定或闭合复位失败者需手术治疗，手术方式有闭合复位经皮穿针内固定、切开复位内固定和肱骨头置换术等，术后（或伤后）规范化的康复治疗和功能锻炼至关重要，甚至比手术本身还更重要。
【诊断步骤】
（一）外伤史与损伤机制
1.最常见的原因是滑倒时用手撑地，暴力沿上肢纵轴向近端传导至肱骨近端致伤。
2.肌肉强烈收缩，如癫痫发作、触电或电休克治疗等，可致大结节骨折，有时可合并肩关节脱位。
（二）临床检查
1.一般情况肩部疼痛、肿胀、活动受限。
2.局部检查①肩部肿胀，可见皮下瘀斑，且在伤后数天更明显，可扩展至侧胸壁。②肱骨近端压痛，有反常活动和骨擦感。③患肢神经血管检查：重点检查腋神经和肌皮神经功能。
3.伴发损伤的检查高能量损伤者注意检查是否有头、颈、胸、同侧上肢或其他部位的损伤。
