Book Abstract

第五节哮喘
小儿哮喘是以发作性的哮鸣气促，呼气延长，不能平卧为临床特征，是一种反复发作的小儿常见的肺系慢性疾病。中医学认为哮喘的内因责之于肺、脾、肾三脏功能不足，痰饮留伏，此为哮喘之夙根。外因责之于气候骤变，寒温失调，感受外邪，接触异物、异味以及过食生冷成酸，活动过度，情绪激动等。内因是发病的根据，外因即诱因是发病的重要条件。哮喘的发生都是外因作用于内因的结果，正虚痰伏，邪阻肺络是其主要病机。其发作期的病机为内有壅塞之气，外有非时之感，膈有胶固之痰，三者相合，闭阻气道，搏击有声，发为哮喘。内因不除，外因屡犯，所谓风有动静、痰有鼓息，导致哮喘时作时止、反复发作。哮喘发作期以邪实为主，缓解期以正虚为主，但亦有发作期、缓解期不明，发作迁延，虚实夹杂的复杂证候。
西医学认为大多数哮喘儿童属于过敏体质，本身可能伴有过敏性鼻炎和（或）特应性皮炎，或者对常见的经空气传播的变应原（螨虫、花粉、宠物、霉菌等）、某些食物（坚果、牛奶、花生、海鲜类等）、药物过敏等。小儿哮喘的先兆症状常有鼻痒、打喷嚏、鼻塞、流涕、眼痒、咽痒、胸闷、干咳等。小儿哮喘易在夜间发作。有的仅有咳嗽，有的典型发作。其原因与吸人过敏源或气温降低等有关。夜间迷走神经兴奋也有一定关系。小儿哮喘的典型发作常有咳嗽、气喘、喘鸣、胸前紧缩感、呼吸困难、呼气延长、端坐呼吸、发绀、大汗、发热等，有的可以咳嗽为唯一症状。严重者可有头痛、头昏、焦虑、神志模糊、两便失禁、嗜睡、昏迷、呼吸衰竭等。如有明显胸痛，要考虑胸痛一侧气胸的可能。哮喘反复发作可导致慢性阻塞性肺疾病、肺气肿、肺心病、心功能衰竭、呼吸衰竭等并发症。
本病相当于西医学的支气管哮喘和喘息性支气管炎。
小青龙汤
【来源】《伤寒论》
【组成】麻黄去节，三两（9克）芍药三两（9克）细辛三两（6克）干姜三两（6克）甘草炙，三两（6克）桂枝去皮，三两（9克）五味子半升（6克）半夏洗，半升（9克）（儿童用量酌减）
【功效】温肺散寒，化痰定喘。
【用法】上八昧，以水一斗，先煮麻黄，减二升，去上沫，内诸药，煮取三升，去滓，温服一升（现代用法：水煎温服）。
【主治】小儿外寒里饮证。恶寒发热，头身疼痛，无汗，喘咳，痰涎清稀而量多，胸痞，或干呕，或痰饮喘咳，不得平卧，或身体疼重，头面四肢浮肿，舌苔白滑，脉浮，指纹淡红。
【方解】本方主治外感风寒，寒饮内停之证。风寒束表，皮毛闭塞，卫阳被遏，营阴郁滞，故见恶寒发热、无汗、身体疼痛。素有水饮之人，一旦感受外邪，每致表寒引动内饮，《难经·四十九难》说：“形寒饮冷则伤肺。”水寒相搏，内外相引，饮动不居，水寒射肺，肺失宣降，故咳喘痰多而稀；水停心下，阻滞气机，故胸痞；饮动则胃气上逆，故干呕；水饮溢于肌肤，故浮肿身重；舌苔白滑，脉浮为外寒里饮之佐证。对此外寒内饮之证，若不疏表而徒治其饮，则表邪难解；不化饮而专散表邪，则水饮不除。故治宜解表与化饮配合，一举而表里双解。方中麻黄、桂枝相须为君，发汗散寒以解表邪，且麻黄又能宜发肺气而平喘咳，桂枝化气行水以利里饮之化。于姜、细辛为臣，温肺化饮，兼助麻、桂解表祛邪。然而素有痰饮，脾肺本虚，若纯用辛温发散，恐耗伤肺气，故佐以五味子敛肺止咳、芍药和营养血，二药与辛散之品相配，一散一收，既可增强止咳平喘之功，又可制约诸药辛散温燥太过之弊；半夏燥湿化痰，和胃降逆，亦为佐药。炙甘草兼为佐使之药，既可益气和中，又能调和辛散酸收之品。药虽八味，配伍严谨，散中有收，开中有合，使风寒解，水饮去，宣降复，则诸症自平。
【临床应用】
1．用方要点小儿症见恶寒发热，无汗，喘咳，痰涎清稀而量多，舌苔白滑，脉浮为本方用方要点。
2．随症加减若外寒证轻者，可去桂枝，麻黄改用炙麻黄；兼有热象而出现烦躁者，加生石膏、黄芩以清郁热；兼喉中痰鸣，加杏仁、射干、款冬花以化痰降气平喘；若鼻塞，清涕多者，加辛夷、苍耳子以宣通鼻窍；兼水肿者，加茯苓、猪芩以利水消肿。
