Book Abstract

可摘局部义齿的支持组织（基牙、黏膜和牙槽骨）属于生物体，并且较易遭受力的作用发生改变。因此，在设计可摘局部义齿修复体时必须注重力作用的方向，持续时间和频率，以及力的量值。因为正常情况下，潜在的破坏力是很小的，支持组织的生理耐受力完全能抵抗这些破坏力而不发生生理或病理变化。但是如果增加这个力的持续时间和频率，那么往往会超过支持组织的生理耐受阈，从而发生病理性改变。从某种意义讲，可摘局部义齿优化设计各部件的放置和均衡咬合能很大程度上分配、分散和降低由修复体产生的不良作用力。

（一）黏膜

黏膜是覆盖于牙槽骨表面的一层软组织，通常在2kg/cm2的压力下会产生0。1～0。6mm的压缩变形。正是黏膜的这种弹性特性，间隙性的殆力不会成为有害刺激。相反，这种力能刺激上皮细胞层的增殖，上皮突起呈指状渗入皮下结缔组织，使两者结合更致密，更能耐受咬合压力。因此，均匀性、间隙性的骀力对黏膜是一种良性的刺激，有利于上皮细胞的新陈代谢。

（二）基牙

天然牙通过牙周膜中的牙周韧带连接悬吊在牙槽内。由于牙周韧带特殊的排列方式，牙齿在垂直向抵抗外力的能力最强，侧向抵抗力较弱。例如，当垂直向作用力与侧向力大小相等时，后者所引起的牙齿的位移将是前者的2～5倍。这是因为牙槽骨受到拉应力时成骨细胞活跃，而受到压应力时破骨细胞活跃。因此，牙齿在受到垂直向作用时牙周韧带受到牵拉牙槽骨受到的是拉应力，牙齿只产生轻微的下沉。而同样大小的侧向力牙槽骨受到的是压应力，牙齿则会发生倾斜移动。其次，每个牙齿的牙周膜面积不同，承受的外力的能力也不同。这就提示我们在设计卡环、骀支托时应采取相对应的措施，如应力中断、压力缓冲、连结固定，追加基牙、基牙联合等措施来分散、均衡骀力。

（三）牙槽骨

牙槽骨由骨密质和骨松质组成。骨松质内有骨小梁，骨小梁的排列与牙的受力方向一致。牙槽窝中上部为横向排列，窝底呈纵向排列，根尖呈垂直排列。它的排列与应力大小密切相关，骨小梁可以随着应力的变化而发生结构排列的变化。正是由于骨小梁结构的特殊性、合理性，使牙槽骨能承受咀嚼器官所给予的复杂外力。
