第1章妊娠合并重型肝炎的基础
第一节肝的解剖
肝是人体内最大的消化腺。肝产生的胆汁经胆管输入十二指肠，参与脂类物质的消化；肝有丰富的血窦，除肝动脉外，还有从胃、肠、胰、脾的静脉汇合而成的门静脉血输入血窦内；肝细胞既形成胆汁排人胆管，又合成多种蛋白质和脂类物质直接分泌人血；由胃肠吸收的物质除脂质外全部经门静脉输入肝内，在肝细胞内进行合成、分解、转化、储存。因此，肝是机体进行物质代谢的重要器官；肝内还有大量巨噬细胞，是清除从胃肠进入机体的微生物等有害物的重要屏障。
一、肝的形态、位置和结构
（一）肝的形态
肝略似楔形，右缘宽大而圆钝，左半部扁薄。肝有膈面和脏面及下、后、左、右4缘（图1-1A、B、C）。
1．膈面膈面对向膈穹窿，其前上面有矢状位的镰状韧带附着线将肝分为左、右叶见图1-1A。后面与上面以左、右冠状韧带前（上）层的附着线为界。左、右冠状韧带前、后层附着线之间形成一三角形的无腹膜包被区，称为裸区。裸区左部分处有一纵行的深沟，少数人可以是穿肝实质的管道，容纳下腔静脉上段，称腔静脉沟。

孕产妇死亡率是衡量一个国家及地区卫生工作水平和发达程度的重要标志，妊娠合并重型肝炎是我国孕产妇死亡的主要原因之一。多年来我们一直把妊娠合并重型肝炎救治作为学科建设的主要方向之一，成立了妊娠合并重型肝炎救治小组，对妊娠合并重型肝炎进行了较长时间的临床观察与探索，得到了省市等有关部门的支持帮助，多次举办过妊娠合并重型肝炎救治的国家级继续教育学习班，取得了一些经验和进步，降低了妊娠合并重型肝炎的病死率。我们在临床实践中发现，妊娠合并重型肝炎的高病死率有其病情复杂危重的原因，也有部分是医务人员缺乏经验，未能早期识别和及时转运，甚至有不适当的产科处理，从而加剧患者的病情恶化甚至导致死亡。目前国内、外尚未见一本权威实用的临床参考书，业内同行学者的呼声日高，于是我们在总结自己的有关经验教训和多次举办学习班的基础上，联络各专业相关专家，历时数年，就临床上妊娠合并重型肝炎救治的方方面面进行了精心撰写。
