3.胃食管反流病有何临床症状？
病人常以烧心、泛酸、反胃、喉部异物感，以及饱胀等消化不良表现、咽下困难或胸骨后疼痛等症状就诊，重症病人可出现食管狭窄的表现，偶见大出血。
烧心是指胸骨后或剑突下烧灼感，常由胸骨下段向上伸延，多在餐后1h出现，卧位、弯腰或腹压增高时可加重。反胃是指在无恶心和不用力的情况下胃内容物涌人口腔，反流物呈酸性者称泛酸。反流的胃液若侵蚀咽部、声带和气管而引起的慢性咽炎、慢性声带炎和气管炎称为Delahunty综合征，有明显的慢性咳嗽或声音嘶哑。胃液反流和吸人呼吸道尚可导致吸人性肺炎、夜间喘息。如已造成反流性食道炎，早期可有间歇性吞咽困难、但无进行性加重。严重食管炎可因食管糜烂而呈慢性少量出血，偶尔可引起急性大量出血。晚期因瘢痕形成可引起永久性食管狭窄。
4.如何诊断胃食管反流病？
有胸骨后烧灼不适、疼痛、反胃等症状，常因平卧或俯腰而加重者，应考虑本病。首选内镜检查，如发现有反流性食管炎表现可确诊。若内镜检查黏膜正常，可酌情选用质子泵抑制剂（PPI）诊断陛治疗试验、X线钡剂检查、食管pH测定、食管测压、核素进一步确诊。GERI）是以烧心、反酸等症状为主要表现的疾病，在询问病史中，对主要症状严重程度和发生频率进行评分。积分多少对GERD诊断也有帮助。2002年我国上海、北京、广州、杭州、西安、武汉和成都等地10家医院借鉴国外经验，设计出反流病问卷调查表。研究发现反流症状评分与食管下端酸反流及食管下端炎症程度呈正相关。
