Book Abstract

神经根和马尾神经的损伤属于周围神经损伤，通常采用三度损伤分类法，即神经失用、轴突中断（axonotmesis）和神经断裂（neurotmesis）。神经失用表现为神经的生理性传导出现障碍，是神经功能暂时性丧失，但没有轴突的破坏，这种情况的预后最好。轴突中断时，轴突纤维只发生微小的破损，髓鞘仍保持完整，损伤的神经将表现WallerTian退变，通过轴突的再生，进行神经鞘的修复。轴突中断恢复期比神经失用长，但预后仍较好。神经断裂时表现为轴突和髓鞘均破损，尽管神经将通过轴突芽孢生长尝试再连接，但严重的损伤断端发生改变并重新排列，使修复变得非常困难，因此预后很差。临床上脊神经根和马尾神经损伤很少出现神经完全断裂的情况。
上述神经损伤分类方法有一定实用性，但临床意义不大。临床常用方法是通过检查肌肉损伤程度和皮肤感觉障碍的范围，将神经损伤分为完全性和不完全性损伤，并通过观察神经功能丧失程度判断预后，如深感觉和运动功能严重障碍提示完全性损伤，预后差。不完全性神经功能损伤纠正后有望自行修复。
临床判断受很多因素影响，相邻的神经根的运动功能有交叉支配现象，因此单一神经根的损伤很少出现完全性麻痹，感觉神经分布区也有重叠现象，很少出现感觉完全消失的临床表现。当怀疑是内固定引起神经损伤时，应结合x线、CT和MRI检查进行综合判断。硬膜外血肿、积液或假性硬脊膜膨出可引起迟发性神经损伤。临床上应根据神经损伤类型，采取必要的辅助检查，进行积极处理。
