王小竞，女，现任第四军医大学第三附属医院儿童口腔科主任，主任医师，教授，博士生导师。
主要学术任职：中华口腔医学会儿童口腔医学专业委员会主任委员，陕西省口腔医学会儿童口腔医学专业委员会主任委员，卫计委执业医师定期考核儿童口腔专业命题组组长，中华医学会医疗鉴定专家库成员，陕西省优生优育学会常务理事，陕西省口腔医学会理事，西安市医学会口腔医学分会常委，东京齿科大学客座教授，国家自然科学基金课题评审专家；<实用口腔医学杂志》《华西口腔医学杂志》和《牙体牙髓牙周病学杂志》编委，多部SCI杂志及《中华口腔医学杂志》等杂志的审稿专家。
研究方向与成果：承担国科金面上项目4项，国际合作及省部级基金13项。发表国际及源期刊论文90余篇，SCI收录期刊24篇。指导30余名博硕士研究生。编译专著6部。自主研发的钛夹板松牙固定链获国家专利1项，并在临床上广泛应用。

Introduction

近年来，越来越多的家长从儿童婴幼儿时期就开始关注其牙齿的排列与咬合。然而，许多口腔正畸专科医师对牙列问题主张不进行早期干预，对来院就诊的患儿多建议观察至12岁左右，在其替换为恒牙列时才开始治疗。这主要基于以下原因：希望缩短疗程；无法判断锚（牙合）畸形未完全暴露前对其进行早期干预治疗是否合适；难以预测乳牙列期和替牙列期牙齿生长发育变化，易造成复发。
儿童12岁时进入青春期，此时，儿童牙列和咬合的生长发育基本完成，第二性征开始出现，但颌面部的生长发育将持续至20岁左右。那么，12岁时开始治疗错（牙合）畸形是否最为恰当？此阶段开始治疗，牙齿移动所需时间相对较短，但复发的可能性较高。因此，在治疗完成后，为了维持治疗效果，需要长时间保持，或切削及磨改牙齿，甚至需要终身佩戴保持器；而切削健康的牙体组织存在易患龋齿等风险。
基于上述原因，到儿童口腔科咨询错（牙合）畸形治疗的家长和患儿逐年增多。笔者主张从乳牙列期就开始预防性治疗错（牙合）畸形。实践表明：乳牙列期开始咬合诱导治疗，患儿适应性较好，不良反应相对较少，经过适当的保持后，复发率相对较低。同时，还可以充分利用不同部位牙颌的生长发育特点，尽早完成治疗，实现有效的咬合诱导。对于已发育完成的牙列，错验畸形的治疗不应仅停留在牙列的形态调整，还要考虑相应口腔肌功能的改善以及较高的复发风险等问题。
