Author Description

李金锋，男，肿瘤学博士，研究生导师。北京大学临床肿瘤学院暨北京肿瘤医院乳腺癌预防治疗中心副主任医师、副教授。
临床上作20余年，经验丰富，具备扎实的外科功底，手术技巧娴熟。目前重点研究乳腺癌的保留乳房治疗、乳房成型、前哨淋巴结活检免除腋窝淋巴结清扫及不同病期乳腺癌的综合治疗。工作中体现以人为本，在不影响治疗效果的前提下，更多考虑女性生活质量的提高，追求乍活的完美。以第一作者身份发表论文50余篇，主编及参编专著8部。

Book Abstract

雌激素影响女性的性生活是通过保持阴道湿润和使阴道延长。未产生性兴奋时，阴道是一个瘪陷的缝隙，而不是想象的管道。在出现性兴奋之后，阴道变长变宽，阴道内膜的细胞会分泌小的“汗滴”以使阴道润滑。阴道的这些变化全是雌激素的功劳，如果一个女性的雌激素水平下降，如绝经期后，她的阴道肿胀和润滑的过程会很慢。没有雌激素，阴道内膜变薄，阴道壁也失去延长的能力。即使她感觉已经非常兴奋，阴道可能仍然紧而干涩，这种状况被称为“阴道萎缩”。
21.1.4女性性高潮的正常形式当女性出现性兴奋时，神经系统将愉悦的信息送达大脑。当信号非常强烈时，激发高潮反射。在高潮期，环绕阴道的肌肉有节律的收缩，肌肉张力的突然释放使愉悦波遍布会阴区，有时可以传遍全身。兴奋过后感觉无限放松从而获得满足。一些女性能够获得多次高潮，一波接一波；另一些人可能在一次性交中只获得一次高潮；还有部分人在性交中未能获得高潮。但所有这些形式都是正常的，女性的性高潮在每次性生活可能都不一样。她可能出现一次非常强烈的性高潮后希望一夜再无性生活；而在另一天，她可能会出现3或4次性高潮。
21.1.5性高潮如何获得性高潮是一种自然反射。然而，多数女性需要学习一些经验来激发它。在性交中获得性高潮常常比对阴道外部的刺激更难些。大约有1/3的女性在性交时如果不对阴道外部区域触摸刺激就不会获得高潮。并非性交产生的性高潮就“好于”其他方式引起的性高潮，而且，同步高潮，即男女同时达到高潮的情形也不是每个人都能达到的目标。
有多种性兴奋的方式可以激发性高潮，每个女性都可以不同。只有很少数女性单纯通过性幻想或触摸乳房就能达到性高潮；也有在睡眠时的梦境中经历性高潮；但大多数女性需要对生殖器的爱抚才能出现性高潮。
女性生殖器对触摸最敏感的部位是阴蒂和小阴唇，许多女性通过抚摸阴蒂可以很容易达到高潮。阴蒂也和男性阴茎一样分为头和体，其功能是在被触摸时将愉悦的信号传至大脑。阴蒂头非常敏感以致于较为快速和强烈的触碰会感到疼痛，为避免疼痛发生可用一种胶或改为直接触摸阴蒂头周围的区域。
对生殖器周围组织，包括大阴唇和肛门周围区域的触摸也可给女性带来快感。每位女性的敏感区域也存在不同。
阴道口处具有丰富的神经末梢，对轻抚的敏感性远胜过阴道深处。对许多女性，阴道前壁比后壁更敏感，一些性治疗专家推荐，对阴道前壁10cm深度以内的触摸可以帮助女性在性生活中达到高潮。
21.1.6癌症治疗过程中及治疗后保持性健康的关键
如果在癌症治疗过程中及治疗后仍想保持性健康，应该注意以下几点：
（1）了解所患癌症的治疗对性生活可能产生影响的全部信息。
（2）应该清醒地认识到，无论你将采用何种治疗手段，经爱抚获得愉悦的能力不会改变。除脑部和脊髓的肿瘤之外，很少有治疗会损伤由触摸获得愉悦和高潮的感觉神经和肌肉。如果女性的阴道紧缩疼痛或干涩，可以通过触摸乳房和外生殖器获得高潮。尽管性活动的某些方式发生改变，但还是能够获得愉悦感。
