Book Abstract

五种病色（赤色、白色）
人体患病的面色可分为赤、白、黄、青、黑五种，分别见于不同脏腑和不同性质的疾病。《灵枢·五色》提及以五色分属于五脏，其对应关系是“青为肝，赤为心，白为肺，黄为脾，黑为肾”。比如病人见青色，可能是肝的问题；见黄色，病位可能在脾；颜色白，可能是在肺；赤，可能是在心。这是五色配属于五脏，根据这个颜色的配属来推测可能是哪一个脏器的病。还讲“青黑为痛，黄赤为热，白为寒”，这是判断病情的性质。从判断病性的角度看，红是热、白是寒，这虽是一般规律，但不可生搬硬套。
赤色
面部或皮肤出现赤色，一般见于热证，偶然见于戴阳证。红色的具体表现又可分为三种：一是满面通红，为实热证。比如发高烧，面部红色明显。多因邪热亢盛，血行加速。中医有句话叫“血得热则行，得寒则凝”，因血热妄行，面部脉络扩张，气血充盈，所以红色面积大而且颜色深。二是两颧潮红，为阴虚证。是因阴虚阳亢，虚火炎上所致。比如肺痨（肺结核）的患者，一般两颧多见红色。还有一些中老年人，每到下午两颧就有一些发红，也往往是阴虚的表现。三是泛红如妆，为戴阳证。戴阳证面色的特点多是面色苍白或晦暗而出现泛红如妆。要注意，平常面色是苍白的或暗淡无光的.并不红，直到病情严重的时候面部时时泛出红色了，这个红色就像是化了妆一样。提到化妆，从古到今基本上都是先“着粉”，后“施朱”。用现在的话说就是，先打粉底，再上腮红。当然，化妆也有个技术问题，化得好的比较自然，化得不好的就像涂上的一样。这样的病人平常的面色是苍白的，而到了病情加重的时候，面部反而现红色，看上去这红好像是涂上去的。这种面色多为戴阳证。那什么是戴阳证？首先说一下“戴”字。戴，本义就是加在头、面、颈、手等处，如戴帽子、戴眼镜等等，这里引申为头面部。戴阳证，是说阳气像戴帽子一样“戴”在了头面部。阳气本应该在哪里？应该在体内脏腑，脏腑中关键在肾，因为肾阳为五脏阳气之根本。那阳气都跑到头面去了，脏腑中特别是肾中已经没有阳气了。往往见于久病肾阳虚衰，阴寒内盛，阴盛格阳，而虚阳上越于头面，病性上属寒，多为病情危重的表现。
看面色还需要注意体质的差异，或生理变异等情况。古人按五行理论，将人按体质不同分为木形人、火形人、土形人、金形人、水形人。其中，火形人面色稍红，特别在夏天面色稍微红一点，都属正常。换个角度说，这个人平常脸色就偏红一些，一直这样，生来就有，也没有其他不适，那现在面红就不一定得了热证。这种体质的人可能是在劳动、饮食、喝热水的时候脸色也要比正常的脸色要红一点，大多是正常。
白色
面部或皮肤出现白色，多为虚证（包括血虚、气虚、阳虚）、寒证、失血证。白色多虚证，什么虚？血虚、气虚、阳虚。但是没有阴虚，阴虚病人不会出现面白，应该是面红，因为阴虚就会阳亢，就会有火热，所以面赤，就是前面提到的两颧潮红。气虚、血虚、阳虚都可以出现面色白，特别是血虚，第一位的是血虚，面部缺少血色而面白；第二位是气虚，因为气能生血、气能行血，气虚不能生血易导致血虚，气虚不行血，就不能够把血运输到面部，所以出现面白。阳虚也可以面色白，因阳虚不能够运行、温煦血液，生寒则凝滞血液，故面白。另一方面，面白还见于寒证和失血证，寒性收引凝滞血脉易致面白；失血则面部血脉缺少血液而面白。
面白又可分为三种：一是面色淡白，往往伴有口唇、舌色淡的，多为血虚证或失血证。这个淡白，是因面部缺少血色而淡，就显得白一些。所以说，血虚是出现面色淡白的直接因素。那淡白可不可以是气虚呢？当然是可以的。所以不要机械地去理解，因为气能生血，所以有可能是气虚不生血导致面白。
