Author Description

程敬亮，男，1964年8月出生。从事放射诊断的医疗、教学和科研工作二十余年，现为郑大一附院放射科教授、主任医师、医学影像专业硕士研究生导师，河南省放射学会主任委员，中华医学会放射学分会全国委员，中华放射学杂志编委。主要研究方向为中枢神经系统影像学。正式发表科研论文百余篇，主译专著3部，主审专著1部，获国家实用新型发明专利2项，获科研成果2项。三年来共申报和承担厅级科研课题2项，省级课题6项。是2004年度河南省杰出青年基金获得者。

第2章 MRA技术
MRl的许多优点使其特别适用于评估血管病变，其主要优点是能无创性显示大范围的血管，并且能提供三维空间信息，即使是经静脉注射对比剂增强时，也仅仅是微创的。一系列三维数据的采集，且可在任意角度或方向获得连续层面，使得以多种方式显示感兴趣血管成为可能。尽管MRA也可显示血管壁以描述疾病的进展状况，但其主要用途仍是血管腔成像。
类似于其他MRl技术，图像采集过程中的人体运动可使MRA采集难以成功。人体运动可能是由于患者不配合，也可能由生理性运动（如冠状动脉成像时心脏的运动）引起；或内脏动脉成像时的呼吸运动引起，甚至血流本身流速非常慢、非常快或血流紊乱时也可降低血管腔的成像质量。
随着MRl设备的不断进展，获得高质量MRA血管像的技术也应运而生。当然，对高质量的MRA
的需求也促使MRl设备不断改进，高质量MRA的获得也印证了新技术的可行性。本章将讨论多种不同的MRA技术及显示MRA的方法。
血流动力学
在MRA中，血流信号强度取决于数据采集过程中血流的速度、方向及其变化（如心跳所致的变化），因此，在对正常和病变状态下的感兴趣血管成像时，成像方法的设计和实施以及成像参数的选择，均需要考虑到感兴趣血管的血流动力学变化。如果不考虑患者的人体运动，当图像分辨力足够高，像素值大于血管腔直径时；或当血流速度足够快，血流通过成像容积仅接受较少的RF激励时；以及当整个心动周期内的血流状态保持稳定时，MRA均能提供准确的血管腔图像。但血流状态的变化既可见于健康人，也可见于病变血管。
收缩期和舒张期血流差别可引起血流状态的变化，例如，下肢动脉在明显的快速顺行性血流后会出现一段较为短小的逆行性血流，随后为较长的舒张期间隔。在体血流方式同样受血管形状的明显影响，即使是健康人的血管也是弯曲的，且有许多分支和分叉，这种外形的不同明显影响血流速度，使通过血管分叉入口处的血管管腔轮廓变平，或弯曲的血管内（如主动脉）的血流呈螺旋状。正常情况下的血流通常呈层状血流，且在每一心动周期重复不变。而在动脉硬化区域，血管壁很不光滑，残存管腔粗细不等，使该区域血流紊乱，如从狭窄处喷射出的血流可伴有涡流，这种涡流在每一心动周期是不同的。
