Book Abstract

探查腹腔。探查肿瘤部位及转移与否，根据不同部位肿瘤的切除要求，逐个离断肝脏韧带。腹腔镜下肝切除术过程中的肝脏各部位的显露，须充分利用肝脏各韧带的提拉。
如果术前影像学检查不能非常明确肿瘤和肝内管道的关系，则用腔镜内超声观察确定并用电凝钩标记肝脏切除线。
解剖第一肝门，预留肝门阻断带，也可根据肿瘤不同部位进行更精确的解剖，进行人肝血流阻断。
腔镜下肝血管阻断技术和肝脏离断技术与开腹手术相似，解剖性切除对腔镜手术要求更高，更应仔细解剖和止血。腔镜下止血比开腹手术难度大，肝断面大针缝合止血不可行，在离断时更小心和更耗时，就意味着肝组织的热缺血时间更长，所以离断前对人肝血流的阻断和控制就有更高的要求，尽量做到切除部位血流局部阻断，切不可为了追求速度而忽略前期的管道解剖，在离断中大量出血经常是中转开腹的原因，而人肝血流的整体阻断时间过长又会造成肝缺血再灌注损伤，导致肝衰发生。
