Book Abstract

腋淋巴结传统解剖学分群如下。
（1）外侧群淋巴结沿腋静脉的内侧排列的腋淋巴结，又称腋静脉淋巴结，在乳腺癌各式手术中清扫该组淋巴结时勿须打开腋鞘，这样可有效地避免术后的同侧上肢水肿。
（2）前群淋巴结位于前锯肌表面、胸小肌下缘，沿胸外侧动、静脉分布，又称为胸肌淋巴结。
（3）后群淋巴结位于肩胛下动、静脉及胸背神经周围，又称为肩胛下淋巴结，在清扫该群淋巴结时注意避免损伤胸背神经及肩胛下动、静脉，结扎切断肩胛下血管的乳腺支，以避免术后出血。
（4）中央群位于腋窝中央的脂肪组织内，是临床体检最易发现的淋巴结群，当上肢内收放松时，可以触及该群淋巴结，这是腋淋巴结各群中淋巴结最大、数目最多的淋巴结群。
（5）尖群淋巴结位于锁骨下肌下内方、胸小肌上缘及内侧、锁胸筋膜深面、Haslted韧带外侧，沿腋静脉排列，其所处的位置是腋窝的顶端，因其位于锁骨下，故又称锁骨下淋巴结，是乳腺癌根治术时必须清除的淋巴结群，其与锁骨上淋巴结相交通。
（6）胸肌间淋巴结位于胸大、小肌之间的血管周围的脂肪内，沿胸肩峰血管肌支分布，又称为Rotter淋巴结。
上述的腋淋巴结分群是按照解剖学的规律划分的，这样的划分对于手术时各群淋巴结的清扫具有指导意义，各群淋巴结之间有着丰富的淋巴干相连接，各群淋巴结累及时均可以汇集到尖群淋巴结，而尖群淋巴结与锁骨上淋巴结、纵隔淋巴结相交通，其淋巴干可直接注入颈内静脉或锁骨下静脉，从而引发锁骨上、纵隔淋巴结转移或血行播散。但这样的分群使术后病理科医师在对手术标本进行病检时将遇到腋淋巴结分群的困难，无法在标本上定位外侧群与前群等，故解剖学分群的临床意义受到限制。从乳腺癌的转移特征以及病理学角度出发的腋窝淋巴结分群日前已广泛应用于国内外的乳腺癌临床，其以胸小肌为标志三分腋淋巴结，胸小肌下缘的所有腋淋巴结属于I组或称下群；胸小肌上缘的腋淋巴结属于Ⅲ组或称为上群；胸小肌上下缘之间的淋巴结属于Ⅱ组或中群，包括胸小肌深面和胸大小肌之间的淋巴结。
乳内淋巴结与腋淋巴结一样，是乳腺癌引流的第一站淋巴结，乳腺的任何一部分均可引流至此，但以中央和内侧为明显。乳内淋巴结滑胸廓内动、静脉排列，其向上通过淋巴下与锁骨上淋巴结相交通，分别注人胸导管（左侧）或右淋巴干（右侧），最终注入颈内静脉或锁骨下静脉。乳内淋巴结向下与肝前上部、膈肌前半部及腹直肌上部等淋巴管网相交通。乳腺的淋巴管伴随着胸廓内动、静脉的穿支进入胸内的乳内淋巴结，乳内淋巴结在1～3肋间较为恒定存在，其所处的层次同胸廓内动、静脉。
