2.互相推诿，不负责任
出现了问题，不是查找原因，而是互相推诿，不敢承担责任。产生的原因大体有四个方面：一是责任不清，稀里糊涂，一旦出了事故，相互推诿，在所难免。二是班组长不敢承担责任。既然是一班之长，就应该大度，是自己的责任，就应该承担；是组员的毛病，要批评，又要分清责任。只有班组长做出榜样，才不会发生相互推诿现象。三是组员怕担处分，不敢担责。做错了事，违反了纪律，甚至出现了事故，就应该查找自身原因，不要总找客观原因，更不能把责任推给他人，这样做就失去了做人的原则。四是辨不清是非，分不清对错，尽管这种现象是少数，但应引起班组长的重视。
3.没有责任，赏罚难分明
没有明确分工，就没有明确责任，安全管理成了一锅粥。假设都是这样的班组，肯定要出事的。因为责任是一种担当，是一种使命。没有责任，一切化为乌有。应该说，有的班组长，在业务上确实是个行家里手，但不善于安全管理，分不清责任，易产生这样的问题：一是班组内部不团结，互相扯皮，内耗太多，影响生产任务完成。二是出了问题，谁都不负责任。后果是严重的，既接受不了经验教训，又易重犯类似错误，很不利于班组健康成长。三是缺乏责任感，当一天和尚，撞一天钟。这是培育优秀工人之大忌。四是易产生惰性，没有上进心，缺乏使命感，很难落实岗位责任制。
4.管理混乱，事故难免
班组管理靠制度，靠责任。如果班组管理混乱，那么制度等于虚设，责任不能落到实处。可想而知，这样的班组，迟早会出大事。我们不妨分析一下，那些事故多的班组，大多管理不到位，制度形同虚设。一般有这样几种情况：一是制度不落实，生产无秩序，经常发生这样或那样的事故。二是责任不到人，安全没保证。先进班组之所以先进，主要是落实责任制，各负其责，勇挑重担，把住安全关口。三是行为管理，靠责任制制约和约束。实际上，安全管理主要是对人的行为管理，如果哪个方面出现漏洞，说明人的行为不规范，不符合要求，当然会出问题。所以说，一旦管理混乱，离发生事故就不远了。
