Book Abstract

FBT与其他疗法的不同之处就在于，SFBT的治疗师尽量利用来访者自己维护自身安全的主意与构想，不考虑来访者所提问题的实质，不考虑来访者是否被迫将寻求治疗当作被关禁闭或承受更糟的惩罚的替代选择。因苏·金·伯格和他的大学同学在儿童虐待与忽视领（Berg &Kelly,2000；De Jong & Berg,1998）做了许多的工作，对许多社会工作者进行了培训，使他们会询问来访者或其他人，包括处于危险之中的儿童一系列问题，引导他们说出为了维持并提高自己的安全等级，在需要的环境下，大部分是在日常生活中，他们想要或者需要做些什么。因为SFBT是基于一个假设，即来访者是他们自己生活的专家。所以我们总是只针对于他们自己所设定的安全等级进行干预，无论这个等级有多低；接着会想办法帮助来访者提高现有的安全等级。因此，这成了来访者自己的解决方法，而不是由外力强加于来访者的干预方式。我们认为告诉来访者该做什么是不尊重他的表现，并有损于他们的尊严。另外，从务实的角度考虑，当他们自己的想法发生了改变，大部分人会在未来尽可能长的时间里去尽可能多地改变自己；而当他们违背了自己的意愿，被迫发生改变时，大部分人会在未来尽可能短的时间里去尽可能少的改变自己。比如，一位心理医生治疗一位想重新恢复一段受虐关系的女士，他或许会不由自主地想把这位女士看成是没有能力为她自己或她的孩子作出正确决定的人，他很可能会屈从于这种想法，因此责备、教导她，并主张她离开这段关系。然而，如果抱着想要知道来访者决定背后的想法的心态来处理同一个问题，治疗师或许会说：“你想回到那个你认为非常危险的环境中去，对于这一点你肯定有一个非常好的理由，我想知道你的这个理由是什么？”这种处理方式让我们以一种完全不同的方式去了解她的“逻辑”思路，她认为对自己和孩子来说最为重要的是什么以及她希望自己和孩子的未来是什么样的。对来访者来说，让她自由选择比告诉她应该做什么更能增强她的力量与信心。对于那些处在受虐关系中的人来说，她最不愿意的就是他人试图把自己的想法或解决方法强加给她。此外，这还意味着原本已经处在非常有失尊严的环境中身心受伤的她又一次受到了侮辱。我们不认为她只能两者取其一，相反，我们认为这是可以两者兼顾的，即她可以在维护尊严的同时也作出有利于自己和孩子的决定。治疗师需要思考如何制定出一个符合来访者的经历及现实生活的详细安全计划。此外，可以通过提出评量问句来有效的表现出来访者的小进步，而不是在她几乎处于极大压力中时，试图让她有巨大的改变。有关于诸如报告儿童虐待这样的社会控制问题呢？在美国的几个州以及许多欧洲和亚洲国家，因苏·金·伯格已经与许多儿童保护机构一起做了大量的工作，她调整SFBT准则，使之更好地广泛应用于合法托管（非自愿）的来访者的干预之中。
